Position Summary: Community of Hope is looking for a passionate and innovative Clinical Nurse Manager. The Clinical Nurse Manager is the clinical lead in the development of patient-centered individual care plans to ensure that care is coordinated with patients across health care providers, settings, conditions, community service providers, and caregivers. This position is also responsible for the overall supervision of the nursing/medical assistants at designated site. This position will be based out of our Conway Health & Resource Center in SW DC. This is a full-time position.
Salary- This position starts at $95k and up based on experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.
Highlighted Duties and Responsibilities: 
  • Ensures high quality of care, smooth patient flow and team coordination for patient visits while following the guidelines of the Patient Centered Medical Home model and/or Birth Center model.
  • Assists with chronic care management by providing care and education to patients. 
  • Responsible for the administration and application of all clinical nursing functions, procedures, and protocols. 
  • Prepares patients for examination and treatment including taking thorough patient history, obtaining accurate vital signs, and verifying the chief complaint.
  • Monitors the functioning of the Vaccine for Children program at the clinic according to CDC and DC regulations. 
  • Assists with coordinating Nurse and Medical Assistant Interns, ensuring proper training of both. 
  • Conducts new employee interviews for vacant or newly created clinical positions.
  • Conducts in-service or refresher training as needed to keep clinical staff up to date on medical information as well as further their medical knowledge.
  • Reviews the performance of team members in a timely manner, including completing 90 day and annual performance evaluations per their due dates, documenting any issues or changes, providing recommendations for professional development and/or training opportunities, and recognizing staff when appropriate.
  • Takes initiative and makes decisions within the scope of work. Assesses situations independently, thinks critically, and takes action to address issues or make useful recommendations while maintaining a solution-oriented lens.
  • Implements controlled substance regulations as necessary (For FHBC and Conway only)
  • Complies with all OSHA and Safety guidelines patient complaints, unusual incident reports, HIPAA and OSHA incidents.
Minimum Qualifications
  • Bachelor of Science degree in Nursing required
  • A current, unencumbered DC Registered Nurse license with current CPR certification required.
  • One year of supervisory experience. 
  • One year working in an ambulatory, primary care or med/surg setting.
  • Care management and patient education experience preferred.
  • Experience working with patients with substance use disorders and behavioral health concerns preferred.
  • Knowledge of chronic disease processes and health maintenance required.
  • Demonstrated cultural competence in communicating with low-income populations required.
  • Ability to work with computers and electronic health records required.
  • Strong verbal and written communication skills required

For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC.
