Job scope: TheCommunity Health Worker (CHW) Program Manager position provides oversightauthority over the CHW Program. The CHWProgram Director will spend, at a minimum, fifty percent (50%) of work timedelivering direct participant services that meet performance measures. Community Health Worker is key in the role ofincreasing opportunities for substance users, including opioid users, to reduceany type of harm related to users' substance use. The CHW will address the following: increase retention of eligible population insubstance use and mental health services, help eligible population address anymedical needs developed from their substance use disorder (SUD), help eligiblepopulation eliminate their substance use disorders (SUDs) and build afoundation for their recovery. The CHWwill work with youth and adults who have substance use issues. The CHW addresses health disparities that adverselyaffect groups of people who have systematically experienced greater obstaclesto health based on their racial or ethnic group; religion; socioeconomicstatus; gender; age; mental health; cognitive, sensory, or physical disability;sexual orientation or gender identity; geographic location; or othercharacteristics historically linked to discrimination or exclusion." 

Education/licensure: Prefer Bachelor's degree in psychology,social work or other substance use related fields; Licensed or Certified as RecoveryCoach, Peer Provider, Licensed Chemical Dependency Counselor (LCDC), RegisteredNurse, or Licensed Vocational Nurse. Will obtain and maintain Texas Department of State Health Services(DSHS) CHW certification. Valid driver'slicense in state of residence and ability to be insured.

Experience/Knowledge: Musthave a minimum of two (2) years of experience in one or more: substance use outreach, substance useintervention; or substance use treatment. Must have a minimum of one (1) year experience in at least two (2) ofthe following: Working with prisonpopulations, working with individuals with housing instability, working withindividuals with SUD, Human Immunodeficiency Virus/Sexual Transmitted Diseases(HIV/STDs), and behavioral health issues, Community health work, or supervisoryexperience. Knowledge of MotivationalInterviewing techniques, service coordination, and crisis management andprevention.

Physical requirements that are bona fide occupationalqualifications essential to the performance of the job: Ability andstrength to sit, bend, and reach during an 8 to 10 hour day. Able to lift a minimum of 10 pounds. Must be able to travel by automobile for upto 6 hours daily. Sensory acuitysufficient to evaluate, treat, and monitor patient needs and maintain accuraterecords. Fine motor skills sufficient tolegibly document and communicate.
