Community Health Center is Hiring a Care Case Manager Near Stillwater, OK
Job Summary:
We are seeking a dedicated and compassionate Care Case Manager to work in our Langston and Carney locations. As a Care Case Manager, you will be responsible for coordinating and providing comprehensive care to patients in need. Your role will involve assessing patient needs, developing care plans, and collaborating with healthcare professionals to ensure the best possible outcomes. Duties:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Collaborate with healthcare professionals, including physicians, nurses, and social workers, to coordinate and monitor patient care
- Advocate for patients' rights and ensure they receive appropriate medical services
- Provide education to patients and their families about treatment options, resources, and support services
- Utilize managed care principles to optimize patient outcomes while managing costs
- Maintain accurate and up-to-date documentation of patient interactions and progress
- Adhere to HIPAA regulations to protect patient confidentiality
- Utilize medical terminology effectively in communication with healthcare team members
Requirements:
- Associate or Bachelor's degree in a related field social work or LPN preferred
- Previous experience in case management or a related field is highly desirable
- Knowledge of hospital systems and processes is beneficial
