The Registered Nurse/Care Coordinator will facilitate the development, coordination and maintenance of Inter-Professional Collaborative Practice (IPCP) teams to deliver quality compassionate patient care. Candidate will utilize nursing process of assessment, planning, implementation, and evaluation of the health system to collaborate, delegate, provide and document care. Improve effective communication among the IPCP team by working closely with Information Technology (IT) department to maximize the electronic health record functions.
