The Registered Nurse/Care Coordinator will facilitate the development, coordination and maintenance of Inter-Professional Collaborative Practice (IPCP) teams to deliver quality compassionate patient care. Candidate will utilize nursing process of assessment, planning, implementation, and evaluation of the health system to collaborate, delegate, provide and document care. Improve effective communication among the IPCP team by working closely with Information Technology (IT) department to maximize the electronic health record functions.

Essential duties and responsibilities include the following. Other duties may be assigned.

Quality Assurance:

• Participates in departments/divisional Quality Assessment and Quality Improvement programs.
• Maintains compliance with regulatory standards regarding practice, environment and documentation.
• Provides continuing education and updates to providers and staff as appropriate.

Clinical practice:

• Performance key targeted physical assessment on patients as determined necessary and/or designated by a provider
• Seeks to understand, incorporate and disseminate both safe practices and evidence-based practices to IPCP teams
• Works with IPCP team to improve population-based chronic disease management to selected CHASS patient populations such as diabetes, obesity and hyperlipidemia
• Performs resuscitative procedures
• Responds to requests from providers regarding resource-finding
• Coordinates Case Management Team meetings
• Functions as the clearinghouse for monitoring and identifying patient issues (i.e. frequent no shows) and aggregate trends (increased numbers of uninsured patients needing services, equipment, etc.…, that is difficult to obtain)
• Develops resources for addressing emerging problems (i.e., low-cost pharmacy lists, dental resources, patient education material, etc.)
• Works with providers to develop action plans based on goals generated from Practice Management System Reports

Research:

• Assists with research protocols where appropriate.

Other:

• Upholds goals, policies, procedures, and expectations of CHASS Center, Inc.,
• Maintains established departmental policies and procedures, objectives, Quality Assurance program, safety, environmental and infection control standards.
• Supports the mission, vision, and values of CHASS Center, Inc.,
• Maintains patient confidentiality by complying with HIPAA standards.
• Performs other related duties as required, such as preceptor for students and new staff as requested.
• Collaboration with members of the health care team and other health professionals

Education and experience:

• Graduate from an accredited school of nursing and current licensure by the Michigan State Board of Nursing as a Registered Nurse.
• Experience working with culturally, racially, and economically diverse individuals in a community-based organization.

Other skills and abilities:

• Ability to speak fluent Spanish is strongly preferred.
