HOMELESS OUTREACH SPECIALIST

Summary:
The homeless outreach specialist is responsible for representing Transitional Living as a respectful, professional, accessible organization that promotes harmonious relationships with clients and internal/external organizations while following guidelines of out PATH grant per COHHIO and SAMHSA.
Job Duties:
Does appropriate applications of crisis interventions, anticipates and prevents crisis as necessary. May personally intervene when necessary.
Finds and reaches out to homeless who may meet SMI criteria.
A condition of employment with Transitional Living, is for you to be in strict compliance with the Code of Conduct and requirements of the Corporate Compliance Plan
Seeks opportunities within the community to train about the PATH Outreach program, mental illness and the homeless population.
Remains current in general knowledge of psychotropic medication, side effects and DSM-IV diagnosis.
Goes into community several times per week to actively engage homeless population and local supports such as homeless shelters.
Completes all intake and tracking paperwork in a timely manner.
Initiates clear and timely communications with other staff and agencies serving the same clients.
Coordinates client’s needs in the areas of ADL, vocation, education, entitlements, transportation, medical services, legal services, social and community resources.
Reports any unusual behavior of consumer to appropriate staff.

Required Qualifications:
Valid Ohio Driver’s License
Proof of auto insurance
CPR and First Aid
Bachelor's degree preferred, however equivalent experience in social work or mental health field may be considered in lieu of a degree
