Community Foundation of Northwest Indiana is Hiring a Nurse Practitioner - ET Nurse(Wound Care experience required) Near Munster, IN

Wound Care experience as an NP required
Must have successfully completed an accredited program for a clinical nurse specialist or nurse practitioner with a minimum of a master’s degree of nursing; license to practice in the State of Indiana with 2 years current experience in wound and ostomy care
Completion of a WOCN Accredited Professional Education Program and WOCN certification preferred
Enrollment in program must be obtained within 6 months of appointment with WOCN certification completed within 18 months of appointment
Documentation of prescriptive authority granted by the State of Indiana and a valid controlled substance certification must be on file in the Medical Staff Services Department prior to ordering of medications
Must successfully complete credentialing and privileging process before functioning as an advanced practice nurse
Demonstration of completion of an advanced physical assessment class
Successful completion of the Community Hospital Personnel and Nursing Orientation Programs.
Completes continuing education as required by certification
Obtains and maintains current CPR certification
Computer literacy with familiarity of Windows and data base management
