Community Counseling and Mediation is Hiring an Outpatient Psychiatric Mental Health Nurse Practitioner Near Brooklyn, NY

PMHNP at Outpatient Clinic (Article 31)

We are seeking part-time or full-time Psychiatric Mental Health Nurse Practitioners (PMHNP) at our Article 31 Mental Health Outpatient Clinics in Downtown Brooklyn and Midtown Manhattan. As a PMHNP, you will provide comprehensive psychiatric care to clients, focusing on assessment, diagnosis and medication management. About us:

Community Counseling and Mediation (CCM) is a renowned and esteemed provider in the mental health field, operating five Article 31 licensed clinics located in Brooklyn and Midtown Manhattan. With a dedicated team of over 100 licensed clinicians, we deliver comprehensive and personalized clinical services to a diverse client population. Our wide range of clinical services includes individual, group, and family counseling, as well as thorough psychosocial and psychiatric assessments and diagnosis. In addition, we offer psychopharmacology services to further support our clients' holistic well-being.

Responsibilities:

  • Conduct comprehensive psychiatric evaluations of clients referred to the clinic, including gathering medical history, assessing mental status, and conducting diagnostic interviews.
  • Develop individualized treatment plans based on thorough assessments, incorporating the client’s specific needs, goals, and strengths.
  • Prescribe, manage, and adjust psychotropic medications as necessary, while closely monitoring the effects and side effects, in compliance with state regulations and best practices.
  • Provide ongoing psychiatric medication management, ensuring regular follow-ups and monitoring treatment progress.
  • Collaborate with a multidisciplinary team consisting of psychotherapists, social workers, psychologists, and other healthcare professionals to provide holistic care to clients.
  • Educate clients about mental health conditions, treatment options, medication management, and coping strategies.
  • Maintain accurate and up-to-date electronic medical records, including progress notes, treatment plans, medication records, and other necessary documentation.
  • Participate in case conferences, staff meetings, and clinical supervision sessions to ensure high-quality care and professional development.
  • Stay current with advancements in psychiatry and contribute to ongoing quality improvement initiatives within the clinic.
  • Adhere to ethical guidelines and legal standards for mental health practice, ensuring client confidentiality and informed consent.

Qualifications:

  • At least three years of PMHNP experinces.
  • Master's degree in Nursing with a specialization in Psychiatric Mental Health.
  • Current and unrestricted state licensure as a Psychiatric Mental Health Nurse Practitioner.
  • Certification from the American Nurses Credentialing Center (ANCC).
  • Solid knowledge of psychiatric diagnoses, psychopharmacology, and evidence-based therapeutic interventions.
  • Excellent interpersonal and communication skills to effectively collaborate with clients, families, and the interdisciplinary team.
  • Ability to work autonomously, demonstrating strong clinical judgment and decision-making skills.
  • Proficient in using electronic medical records (EMR) and other relevant software systems.
