Community Counseling and Mediation is Hiring a Family Peer Advocate Near Brooklyn, NY

CCM works to improve the lives of at-risk children and families through innovative behavioral health, supportive housing, education, and youth services.

Our expanding network of licensed outpatient mental health and chemical dependence clinics, award-winning housing facilities, HIV and hepatitis prevention and care coordination, and after-school and child-welfare programs, utilize evidence-based, culturally competent practices to achieve positive outcomes and keep families together.

Family Peer Advocate Job description

Responsibilities/Duties

  • Will assist families with identifying needs, strengths, challenges, and goal and develop individualized care plans (ICP)
  • Monitor the family’s progress in achieving goals detailed in the ICP
  • Facilitate groups/workshops and hold individual one-on-one peer support sessions focusing on Engagement, Bridging and Transition Support to help increase parents/caregivers strengths to navigate systems and better meet their children social, emotional and/or developmental needs.
  • Provide parents/caregivers with the tools needed in order to learn how to self-advocate and manage systems.
  • Encourage and support the family’s active involvement in their treatment plan and use of community resources.
  • Coach and model shared decision-making and skills to support collaborations.
  • Accompany families to meetings/school and appointments as needed.
  • Support and encourage family and youth participation in community, regional state, national activities to develop their leadership skills. Encourage civic stewardship.
  • Provide groups and workshops on Parent Skill Development teaching Parents/Caregivers how to learn and practice strategies that encourage positive behavior.
  • Provide education on wellness and resiliency.
  • Encourage Parents to interact and connect with their peers and strengthen relationships with their families.
  • Connect Parents/Caregivers to support systems within their own community.
  • Conduct outreach activities and participate in community events.
  • Make referrals for needed concrete services and additional supports needed.
  • Will help develop clients’ support systems and integrate them into community services.
  • Document and report on all outcomes and goals of the program Supervisor and Alliance Coordinator.

Qualifications

  • Must have a minimum HS diploma or BA
  • A parent/caregiver of a child who has experienced social, emotional, developmental, substance use, and/or behavioral challenges and has ‘lived experience’ navigating a child serving system on behalf of their child(ren) with social, emotional, developmental, medical and/or behavioral needs.
  • Must have basic computer skills and knowledge of office applications.
  • Bilingual in English and Spanish preferred.

Hours of Operation Flexible

9 AM to 5 PM Monday through Friday. May work some evenings, weekends, and Holidays.
