Community Connections is Hiring a Program support assistant Near Durango, CO

We are seeking a qualified and dynamic candidate for this new and exciting opportunity! The candidate may have some remote work but will primarily be based out of the Durango office and needs to have ties to the Durango community.

Attach a current resume and letter of intent. Wage is $17.86 per hour. Tenure with Community Connections (CCI) will be considered for active CCI employees.

Reports To : Employment Support Program Manager

FLSA : Hourly / Non-exempt

Job Description : This position supports the Durango Employment Support department. Time will be distributed between direct service provision and management of specific requirements as defined in the job description.

Will be responsible for the day-to-day operations of the Durango Employment Support department under the management of the Employment Supports Program Manager.

A flexible schedule can be established primarily working Monday Friday but is required to meet the support needs of people receiving services and program scheduling.

Responsibilities :

  • Advances the mission, vision and values of Our Own Lives while performing the responsibilities of this position.
  • Complete and maintain certifications required to meet DVR and HCBS requirements to provide job development, job coaching, and customized employment services.
  • Market Our Own Lives and its participants to potential employers in the community in a variety of settings.
  • Develop weekly staff schedules in coordination with Employment Support Program Manager.
  • Learn and follow State regulations and service definitions.
  • Provide Job Development and customized employment services for the Durango area as a primary responsibility.
  • Provide Job coaching as needed.
  • Work with Employment Support Program Manager as the liaison between HCBS-SLS, HCBS-CES, and HCBS-DD Waiver services and the Department of Vocational Rehabilitation (DVR).

This includes assisting the client with DVR performing job placement activities and assisting the client to transition from DVR to HCBS-SLS, HCBS-CES, and HCBS-DD Waiver services funded vocational supports.

  • Assist the IDT in the development of the annual Service Plan.
  • Develop and oversee the implementation of all vocational ISP goals and assessments.
  • Coordinate contracted employment support as requested by DVR for their clients that are not otherwise part of Our Own Lives services.
  • Assist clients with DVR application process.
  • Assist clients with resumes and update accordingly.
  • Complete and submit invoices, monthly progress reports, and other activities necessary for billing of DVR services.
  • Attend community events when requested to promote business connections.
  • May perform other assigned duties related to administration of Durango Adult Services. These duties may include support with scheduling, billing, updating documentation, maintenance of Therap and other files and records.
  • May fill in for other Direct Support Professional staffing requirement as needed.
  • Participate in rotation of on call phones, including nights and weekends.

Qualifications :

Physical Demands of the Job : These essential tasks may be performed with or without accommodation, or with assistive technology.

While performing the duties of this job, the employee is regularly required to be stationary, move indoors and within the community, position self, use objects, communicate verbally, in writing, and via in-person, computer and phone, and observe.

The employee may be required to lift heavy weights up to and over 50 lbs., such as assisting and transferring individuals independently, two persons, or with adaptive equipment from wheelchairs, beds, showers, etc.

The employee may be required to travel independently or with passengers to meetings, training, and other community activities.
