Layton Blvd, Milwaukee, WI 53215. Licensed Practical Nurse/LPN Job Responsibilities: Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday operationsServes as coordinator of care for scheduled weekend and holiday shiftsEnsures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin. Licensed Practical Nurse/LPN Job Requirements: Education: Diploma from accredited program in practical nursing requiredLicensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing Certification: Current CPR/AED certifications Licensed Practical Nurse/LPN Job Schedule: Work Schedule hours: 6:30 a.m. – 3 p.m. (40 hours per week)Every other weekendEvery other holiday Community Care is a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible.
