Community Care is hiring a part-time Physical Therapist for our PACE program in Racine and Kenosha counties!This position is a part of the managed care program called the program for the all-inclusive care for the elderly (PACE). As the only PACE provider in Wisconsin, this is a unique opportunity to provide care through direct access to all our members without the worries of insurance verification or billing. Daily work incorporates a combination of direct care/treatment, team meetings, and remote management of member needs. Our rehab team focuses on keeping members in the least restrictive living environment through proactive interventions, chronic disease management, and injury prevention. Monitors and manages care for our members rehabilitative needs, including DME and home safety.Physical Therapist Job Requirements: Education: Bachelor’s degree in physical therapy from an accredited program required.Experience: Minimum of one year of personal, clinical/internship, and or employment experience providing care or services for adults with disabilities or frail elderly required.License/Certification: Current State of Wisconsin PT license and CPR & AED required.Travel: Requires use of a personal vehicle and valid driver's license for home visits and travel between Community Care sites.Physical Therapist Job Schedule:Part-time; 32 hours per week Monday to Friday8 a.m. to 4:30 p.m.Flexible working scheduleAbout Community Care:Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community.
