This is a full-time opportunity to service Dane County, Jefferson County, Rock County, and Walworth County.Behavioral Health Specialist Job Responsibilities: Responsible for providing Behavioral Health (BH) services across Community Care Inc. programs for designated members.Serves as a resource to the Interdisciplinary TeamProvides targeted Behavioral Health Case Management services, as assigned. Behavioral Health Specialist Job Requirements:Experience: A minimum of 3 years of experience in providing social work and/or behavioral health case management services to older adults and adults who are physicallyand developmentally disabled required; Mental health experience, including provision of individual counseling and support group facilitation with older adults is preferred.Behavioral Health Specialist Education:Masters Degree, Social Work, Counseling or related field required.Certifications/Licensures: Wisconsin Licensed Advanced Practice SW required; LCSW or LPC preferred. *Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
