Mental Health Agency now hiring experienced individuals to do Youth Outreach for the 16-20 year old population for the below counties.

Alamance, Alexander, Alleghany, Ashe, Avery, Buncombe, Caldwell, Caswell, Chatham, Cherokee, Clay, Franklin, Graham, Granville, Haywood, Henderson, Jackson, Macon, Madison, McDowell, Mitchell, Person, Polk, Rowan, Stokes, Swain, Transylvania, Vance, Watauga

Experienced with program development.

Experienced with outreach with the teenage population.

Complete intake paperwork and care coordination.

Experienced, detail-oriented, professional who is great with paperwork and a community liaison and can adhere to the turnaround time.

Must have own transportation.

Proven results.

1. Assist with the program development and design of training modules for staff within the agency and administer staff training, as needed
2. Completion of the intake/referral form with the individuals being served.
3. Provide input into person-centered plan modifications, as needed
4. Coordinate activities within the established plan; facilitate plan meetings;
5. Assist with referral linkage to other services and community resources
6. Facilitate relationships and serve as a link between the agency, parents, guardians, natural supports, other service providers, and the community;
7. Facilitate and/or participate in treatment team meetings (if indicated)
8. Ensure that all documentation is accurate and reflective of services and completed on time as defined by the agency's policy and state guidelines
