Community Agency for Senior Citizens is Hiring a NY Connects Specialist Part-Time NY Connects: Staten Island Near Staten Island, NY

Purpose and Objectives: Provide objective and unbiased information and assistance on all long-term care options, services/supports and provide follow-up to clients, professionals and caregivers.

Organizational Values: The Community Agency for Senior Citizens (CASC) assists Staten Islanders age 60 and older to live as independently as possible in the community. We do this by working in hand-in-hand with older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage, and recognizing the importance of friends, relatives and community in creating supportive environments for people as they age.

NY Connects is a state–wide program providing objective and unbiased information and assistance to individuals with disabilities of ALL ages with long term care needs or questions, to obtain free and objective information about long term care services and supports over the phone, via email, and in person (onsite and offsite). Information/assistance can be sought by individuals themselves or their caregivers, friends, neighbors or helping professionals.

Position Qualifications:

  • BSW, BS, BA level education is required. MSW preferred.
  • Must have work experience in health/human/social services working with older adults, developmentally disabled, physically disabled, behavioral health and/or substance abuse populations.
  • Familiarity with Medicare, Social Security and Human Resources Administration NYC is preferred.
  • Knowledge in Microsoft Word, Excel and Outlook is required.
  • Bi-lingual (Spanish-English) preferred, but other languages are welcomed as well.
  • Must have a car for travel for work.

Requisite Skills and Capabilities:

  • Knowledge of Microsoft Office, including Word, Excel, Outlook required.

  • Ability to work under the direction of others.

  • Ability to work with considerable self-reliance.
  • Car required for travel for work assignments.

Responsibilities:

  • Be responsible for providing objective and unbiased information and assistance on all long-term care options, services/supports and provide follow-up to clients, professionals and caregivers.
  • Conduct No Wrong Door screening with individuals over the phone, in-person, in order to obtain information for both possible programmatic and financial eligibility for various programs.
  • Application and enrollment assistance for public benefits, including Medicaid as appropriate, SNAP and HEAP.
  • Conduct home visits to assist with benefits/entitlements as appropriate.
  • Will provide Person Centered Counseling when appropriate.
  • Will provide assistance with Care Transitions as appropriate.
  • Will follow all NYC DFTA Program Standards.
  • Maintain case records on NYC DFTA STARS Client Database for all clients and complete necessary statistical documentation.
  • Will participate in mandated State/DFTA funded trainings and any additional trainings as assigned.
  • Other duties as assigned.
  • Some evening and weekend hours will be required.

Supervision and Education:

  • Supervised by NY Connects Program Director.
  • Participate in team meetings.
  • Participate in DFTA and CASC trainings and education sessions.
  • Participate in local educational and informational sessions and conferences.
  • Annual personnel review.

  • Empower clients.
  • Effective referrals.
  • Effective documentation in STARS.
  • Effective implementation of CASC procedures and guidelines
