You'll join a team of talented and passionate people who are motivated to make our community as strong and resilient as possible.

HOURS: Monday – Friday, 25 hours per week.

POSITION SUMMARY:

Ensure availability and accessibility of WIC, Commodity Supplemental Food Program (CSFP), Senior Farmers Market Nutrition Program (SFMNP) benefits to current and prospective participants by performing eligibility determinations in accordance with Agency, State, Federal Policies and Procedures. Completes anthropometric measurements and hematological screening for program participants.

PROGRAM RESPONSIBILITES:

  • Determines participant eligibility for WIC/CSFP/SFMNP program services in accordance with Agency, State and Federal guidelines by completing the intake process by gathering required demographic and financial information to assist with eligibility determination process for all participants.
  • Documents relevant data required for income, residence and other eligibility criteria as determined by policies and procedures and enters information into the StarLINC system.
  • Monitors and assists with completion of appointments through the New Hampshire Easy and WIC online pre-application portal systems.
  • Provides information to participants on approved local vendors accepting e-WIC.
  • Assists in performing hematological assessments and accurately records result in StarLINC System in accordance with policies and procedures.
  • Completes anthropometric measurements and accurately records result in StarLINC system in accordance with policies and procedures.
  • Insures clinic flow is maintained by assisting other staff as needed with intake eligibility process.
  • Assists with set up of each clinic and transports necessary records and equipment to effectively operate clinics as necessary.
  • Refers to appropriate health and social service resources as needed and documents referrals in StarLINC system.
  • Schedules appointments for new participants and follow-up appointments for those already enrolled.
  • Completes appointment reminder calls and follow-up on no-shows by telephone and/or writing to reschedule participant appointments.
  • Assists with preparation of mailing appointment notices and termination letters.
  • Assists with general office duties including answering phones and other clerical duties as needed.
  • Works in agency warehouse or other CSFP/SFMNP sites as assigned to complete CSFP/SFMNP eligibility process for participants.
  • Accurately documents participant information in the StarLINC computer system and manual systems for CSFP/SFMNP.
  • Responds to walk-in or telephone requests for information or services about WIC/CSFP/SFMN, CAP or other community social and health services.
  • Develops and maintains knowledge and working understanding of StarLINC system and its applications and completes necessary computer procedures to maintain StarLINC and equipment as required by state.
  • Develops and maintains knowledge of New Hampshire Easy, NH WIC Pre-application, WIC Smart, Teletask and other portals or systems to support WIC participants.
  • Develops and maintains knowledge and working understanding of the requirements for documentation for CSFP/SFMNP to ensure program compliance.
  • Required to attend staff meetings and other educational training and conferences for professional development.
  • Acquire and maintain knowledge of Agency, State and Federal Policies and Procedures.

  • Present a professional and positive image as a representative of CAPBM.
  • Follow established policies and procedures and comply with all safety requirements.
  • Communicate proactively with the supervisor regarding workflow, problems, suggestions, etc.
  • Contribute to a positive, team-oriented work environment; and attend team and staff meetings as scheduled.
  • Be punctual for scheduled work, use time appropriately, and complete work in a timely fashion with minimal errors.

Requirements

MINIMUM QUALIFICATIONS:

Experience:

  • Two years of experience working in social services and/or clinical environment.
  • Ability to communicate effectively with people of varying economic, cultural and educational backgrounds.
  • Ability to work effectively as part of a health care team; accept direction; and work independently.
  • Knowledge of and ability to use office software applications and computer.
  • General office skills (phone, filing, information processing).
  • Ability to protect clients’ confidentiality.

Education:

  • High School diploma or equivalency credential.
  • Certification as a medical assistant is preferable.
  • Required to maintain annual training requirements including anthropometric, hematologic including use of Hemocue and Lead testing equipment.

License/Certification:

  • Successful completion of criminal record check.
  • Valid driver’s license with access to reliable transportation for required travel.
