Community Action Partnership of San Luis Obispo is Hiring a SSVF Case Manager Near San Luis Obispo, CA

Position Summary:
Under the direction of the Program Supervisor, the Supportive Services for Veteran Families (SSVF) Case Manager provides comprehensive case management, referral, and supportive services to qualified veterans and their families. The SSVF Case Manager works directly with eligible participants, SSVF team members, partner agencies, and landlords/property managers countywide to develop, arrange and coordinate case plans towards the goal of independent housing. Supports CAPSLO mission by providing direct services to SSVF clients in an effort to assist them in self-sufficiency and obtaining independent housing.
Required Education and Experience:
  • Bachelor’s Degree in Social Work, Psychology, Family Studies, Human Services or related field and four (4) years of experience providing direct client services such as assessment, evaluation, treatment planning, referral and follow-up to a population comprised of homeless individuals or families, Veterans, parolees or probationers, the mentally ill, the chronically ill, or high risk youth.
Preferred Education and Experience:
  • Veterans with other than dishonorable discharge preferred.
License & Certifications:
  • Must have dependable, insured transportation, and a valid California Driver License (mileage to be reimbursed) and acceptable driving record.
  • A DMV printout and proof of insurance will be required.
Essential Functions:
  • Conducts a needs assessment to identify vulnerability and make referrals for services.
  • Develops a Housing Stability Plan with goals, responsibilities and timeframes based upon prioritized problems list.
  • Reviews and documents updates for Housing Stability Plan on a monthly basis.
  • Provides relevant referrals to agencies and service providers based upon the goals and objectives mutually agreed upon with the client. Advocates for clients with agencies and service providers to minimize barriers to clients receiving needed services. Follow up with referrals to ensure that clients receive and access necessary services in a timely manner.
  • Assists client(s) in developing a savings and budget plan in preparation for eventual independent living.
  • Coordinates services with other SSVF/non-SSVF service providers including: legal assistance, alcohol/drug and mental health providers, job skills training providers, income stabilization and support providers, and benefits providers.
  • Communicates progress towards independent living with community partners, when appropriate.
  • Works with community partners and Housing Placement Specialists to support efforts at locating and maintaining supportive permanent housing.
  • Maintains a professional relationship with clients and conducts all business related activities according to program policy and standards relevant to the case management field.
  • Monitors client behavior regarding compliance issues and follows-up.
  • Provides consistent limits and expectations regarding program policies and goal achievement.
  • Maintains client confidentiality in accordance with program policies and applicable laws.
  • Re-evaluates client needs and Housing Stability Plan, at minimum, on a quarterly basis to determine if goals are being met. Develops new strategies and/or modifies previous goals as appropriate.
  • Completes all required logs, timesheets, reports and client documentation as required. Maintains statistical information for each individual/family on a weekly basis.
  • Maintains a working understanding of the readjustment problems of veterans including; Housing Barriers, Post Traumatic Stress Disorder, Traumatic Brain Injury, alcohol and drug dependency, and other mental health issues.
  • Conducts home visits to provide support and identify possible referrals needed to sustain permanent housing.
Additional Responsibilities and Duties:
  • Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices; and act in such a manner as to assure at all times maximum safety to one’s self, fellow employees, clients and children.

Knowledge, Skills and Abilities:
Knowledge:
  • Must have knowledge of local human services delivery system.
  • Must be computer literate with basic knowledge of Microsoft Word, Excel and data tracking programs.
Skills:
  • Must have strong verbal and written communication skills.
  • Must have crisis intervention, de-escalation, and conflict resolution skills.
Abilities:
  • Must have experience counseling and ability to motivate and problem solve.
  • Must be able to handle several ongoing projects and have demonstrated organizational skills in former positions.
  • Must have the ability to show warmth and appreciation of human growth at all levels and understand the process of gaining trust and credibility with high-risk individuals and families.
  • Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual orientation, disability or socio-economic level.
  • Must be self-motivated and able to work independently and with minimal supervision.
  • Must understand, uphold, and continue working toward accomplishing the mission, strategic goals and performance measures of the agency.
Supervisory Responsibility:
  • This position has no direct supervisory responsibilities.
Work Environment:
  • Requires working indoors in temperature-controlled environment with some exposure to copier and printer chemicals/fumes.
Physical Demands:
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
Physical:
  • Position requires intermittent sitting, standing, walking, twisting, and bending.
  • Must be able to lift 30 pounds.
  • Simple grasping and hand manipulation required, as well as reaching above and below shoulder level.
Travel:
  • Will require frequent travel within the San Luis Obispo County area with occasional travel to Central Headquarters in San Luis Obispo.
  • Must have a valid California Driver’s License, and a reliable vehicle available for business use (mileage reimbursed) and acceptable driving record.
Additional Eligibility Qualifications:
  • Must be at least 18 years of age by date of hire.
  • Must comply with the CAPSLO COVID-19 Vaccination Policy, either being fully vaccinated or sign a COVID-19 vaccination declination form.
  • Based on program, a current TB skin test/Chest X-Ray may be required.
  • Employment is contingent upon meeting all applicable job requirements and background requirements. A criminal history clearance is required prior to employment.
  • Upon hire, agency staff will check the Megan’s Law (or the current sexual predator website) to ensure each staff being hired is clear for employment. Agency staff will check the website annually to ensure all program staff are cleared to work.
