Position Summary.

As a Homeless Outreach Worker you will be at the forefront of our mission, engaging with individuals experiencing homelessness directly. Your primary responsibility will be to develop strong and trusting relationships with our target population, creating a bridge between them and the resources and services available. The Homeless Outreach Team will provide an intensive team to engage and provide services to homeless adults with serious mental illness at risk of acute or long-term institutional care. The program aims to help adults with serious and persistent mental illness succeed in the community with sufficient structure and support. The Homeless Outreach Team will provide a full range of services, including assessment, individualized planning, case management, integrated co-occurring drug and alcohol services, medication, housing, integrated vocational services, and access to clients on a 24/7 basis. The Homeless Outreach Worker will work as a member of the Homeless Outreach Team to manage a caseload of adults with serious mental illness who have been recently housed. The Homeless Outreach Team will be responsible for crisis care, case management, individual rehabilitation activities, day-to-day skills building, transportation to appointments, and all other aspects required to help the clients maintain stability.
Hours/Salary
Full-time, 40 hours per week. This position includes a generous individual benefits package that includes health, dental, vision, life, 403b retirement (with up to a 5% employer match) paid vacation/holiday time, paid sick time and a variety of helpful employee perks as well. Anticipated hiring wage is $23.00-$27.50 per hour depending on education and/or experience. (*Full Salary Schedule is $23.00-30.50 per hour)
Experience and Skills:
Minimum Requirements
BA/BS AND 6 months of full-time equivalent experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time equivalent experience with target population, OR 4 years of full-time equivalent experience with target population. Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure. Must have a strong commitment to the right and ability of each person with a severe and persistent mental illness to live in a safe living environment; develop a meaningful role in the community; and have access to competent and continuous supports and services. Must have a strong commitment in clients’ rights to self-govern and create their own choices. Strong interpersonal skills, collaborative work style and strong oral/written and communication skills. Able to deal effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level. Well defined organizational and time management skills. Ability to work independently and flexibly. Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices while acting in such a manner as to assure at all times, maximum safety to one’s self, fellow employees, clients, and other persons contacted through TMHA business. Must rotate with other FSP team members to be on-call after normal business hours as needed. Must have reliable automobile and utilize own vehicle at work with a company mileage reimbursement when a company vehicle is not available. Conditions of employment include: Must be at least 18 years of age; possess a valid CA Driver’s License, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance. Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees. Ability to possess and maintain good physical and mental health. Comply with a T.B. skin test or chest x-ray indicating a negative tuberculosis result and receipt of a criminal record clearance through fingerprinting. Must be able to successfully complete First Aid and CPR training.
