All staff in positions which regularly require driving either company or personal vehicles while performing work on behalf of Community Action are required to provide Human Resources proof of a valid driver's license for a motor vehicle record check both at hire and annually thereafter. Human Resources will conduct a motor-vehicle record. These records will be held as confidential records in Human Resources. All accidents will be reported to Human Resources within 48 hours.

Position Summary:

In support of the Agency's mission, and under the direction of the Coordinated Systems Manager, the Homeless Access Systems Coordinator coordinates the daily operations of the countywide Coordinated and Centralized Assessment System - Community Connect. This position actively contributes to a welcoming and inclusive team environment and helps lead program decision-making to ensure culturally responsive, person-centered, and trauma-informed service delivery. Responsible for day-to-day supervision, training, and support to Community Resource Advocates who conduct comprehensive housing assessments, gather the required documentation and provide direct referrals to a variety of Washington County housing programs. Responsible for ensuring screenings, assessments, and referrals take place in accordance with the policies and procedures of the countywide centralized assessment system. Provide training support and technical assistance to community partners conducting Community Connect assessments. This position ensures that client information is accurately entered into statewide client databases. Responsible for working with the program's leadership team to develop new policies and procedures in response to changes in program funding and/or requirements.

Essential Functions and Responsibilities:

  • In partnership with the Coordinated Systems Program Manager and other program leadership, develop, implement, and manage Coordinated Equitable Access to the countywide Coordinated and Centralized Assessment System - Community Connect.
  • Support the coordination and maintain positive relationships with Coordinated Entry Systems partners including the Housing and Supportive Service Network.
  • Promote coordination with other Community Action programs in order to facilitate client access to all applicable funds and services.
  • Meet regularly with intake staff to provide supervision, training, and support, encourage professional development, identify performance concerns, evaluate work, and provide feedback. Responsible for building an effective team by supporting and responding appropriately to concerns and facilitating constructive resolution to team issues.
  • Directs daily operations of screening and intake process to ensure quality in customer service, client-centered, and consistency in operations.
  • Coordinates efficient screening and assessment scheduling to appropriate county locations to ensure equitable access to clients. Provides screening/assessment services as needed.
  • Ensures that staff members understand and comply with established CARE assessment system, policies, and procedures, and provides training and assistance to staff in the provision of client-centered, trauma-informed and inclusive service interactions.
  • Monitors daily data entry activities and ensures adherence to established data entry protocols. Responsible for monitoring data quality and ensuring data quality of staff.
  • Assist or delegate staff in representing the program on local and regional committees, meetings, huddles and workgroups as assigned.
  • Responds appropriately to client issues and concerns as needed.
  • Communicates current program policies and procedures to program and agency staff, outreach partners, community organizations.
  • Provides training and technical assistance to community outreach partners conducting mobile screenings and assessments.
  • Coordinates with the Washington County Supporting Housing Service Team, Community Connect Oversight Committee, Washington County Shelter Systems to ensure that Community Resource Advocates provide appropriate services and referrals.
  • Responds appropriately to client issues and concerns as needed.
  • Strictly adhere to all safety policies and procedures at all times and respond/correct any at-risk safety issues immediately.
  • Strictly adhere to agency code of ethics and standards of workplace behavior.
  • Perform other duties as assigned to meet business needs including regular and reliable attendance and adherence to all company workplace behavior standards.

Required Education/Training/Experience:

  • Bachelor's Degree in Social Services or a related field in addition to at least two (2) years of experience delivering direct services to a diverse population using appropriate customer service techniques, including experience interviewing clients/customers and assessing needs, as well as providing appropriate information and advocacy to meet customer needs. An equivalent combination of education and experience may be considered.
  • Ability to assign, coordinate and supervise the work of others.
  • Intermediate level computer proficiency in word processing, databases, spreadsheets, PowerPoint, and calendaring programs.
  • Proven ability to work collaboratively with a wide range of community members, including professional organizations, service providers, and client advocates.
  • Proven ability to work in an environment requiring frequent problem solving and decision making.
  • Experience working with complex regulations and guidelines requiring implementation, documentation and monitoring for quality control.

Preferred:

  • Prior direct supervisory experience.
  • Knowledge of Washington County community resources.
  • Bilingual Spanish/English, verbal and written.

Required Physical Demands of Essential Functions and Responsibilities:

  • Work is performed in normal office environment.
  • Constant exposure to computer and keyboard with extensive phone use.
  • In normal work day, may stand/walk 4 hrs/day; may sit 4 hrs/day; may drive 1/2 hrs/day; may use hands for repetitive grasping, pushing and pulling, typing and fine manipulation 2 hrs/day; may use hands at or above shoulder level for 1 hrs/day.
  • In normal work day, may bend, squat, and/or climb frequently.
  • Work is light in nature, lifting maximum of 20 lbs with frequent lifting and/or carrying of objects weighing up to 20 lbs.
  • May require walking or standing to a significant degree, or sitting most of the time with a degree of pushing and pulling of arms and/or leg controls.
