Communitas is Hiring a Team Leader/Direct Support Professional Near Bremerton, WA

Responsibilities:

- Provide direct care and support to individuals with developmental disabilities or other special needs.

Please note that this job description is not exhaustive and additional duties may be assigned as needed.

The Team Leader is responsible to assure the needs, wants and desires of the individuals supported are being met. The Team Leader is responsible to assure that their respective team works cohesively while at the same time embodies the vision and mission statement, core values and code of ethics of Communitas.

Duties

Essential Duties and Responsibilities: Being a member of SEIU 775, union dues.

o This position will include personal care, ex.to include showering, bathing, personal hygiene, incontinence care, use of medical equipment, medication passing and the ability to lift and transfer individuals. Transportation to and from medical appointments and or goal completion of individual(s). Using your personal vehicle with mileage reimbursement is a condition of employment. Covering shift(s).

o Welcoming new Team Members/Mentoring new employees.

o Participate in interviews for those applying for permanent positions.

o Evaluation of team members and accountability for progress.

o Be acquainted with household activities, shift responsibilities and schedules of the individuals being supported.

o Participate in and model to team members the integration of residents in the community.

o Relationship development/positive contact with families, guardians, employers and natural supports.

o Facilitate effective team communication and monthly team meetings and monthly team leader meetings.

o Facilitate and monitor attendance of Direct Support Professionals in both their daily schedules and mandated staff meetings.

o Facilitate division of responsibilities by delegating tasks and setting deadlines.

o Scheduling staff to ensure all shifts are covered with qualified staff. Posting any and all open shifts at the location in house and via company e-mail. following the CBA.

o Understanding, utilizing and updating the scheduling tool to meet the needs of the program.

o Monitoring the Direct Support Professionals PTO / sick leave usage to ensure it meets the needs of the program.

o Participation in all phases of Person Centered Planning and Individual Instruction & Support Plans.

o Reviewing T-Logs, IISP Goals, and Clinician Reports for timeliness and accuracy.

o Review daily money counts are being done and financial transactions, to include monthly reconciliations are accurate and turned in timely.

o Budget development and upkeep.

o Ensure medication procedures are understood and followed per policy and protocol.

o Review Medication Administration Record and all medication packets for accuracy(audit). Contact the pharmacy for any discrepancies, questions and reordering of medications.

o Ensure medication audits occur daily.

o Ensure the team is following Communitas Incident Reporting procedures in Therap including guardian notification.

o Reviewing timesheets for accuracy and approving them.

o Utilizing written verbal warning coaching feedback form for the purpose of team problem solving.

o Participate in the development and revision of and understand each resident’s Positive Behavior Support Plan.

o Coordinate gathering data for revisions to Positive Behavior Support Plans and recommend necessary revisions.

o Track, schedule, and coordinate individual’s doctor’s appointments.

o Assure staff is prepared for routine medical appointments and there is proper documentation of the appointment.

o Modeling interactions with individuals supported, assuring empowerment of the individual and flexibility in actions to their needs desires and wants.

o Quality Assurance: Ensuring that the overall care for each individual improves their quality of life.

o Set clear team goals.

o Oversee the day-to-day operation of their assigned program(s).

o Discover training needs and provide coaching.

o Anticipate problems and problem solve precautions to avoid such things from happening in the future.

o Have the ability to rotate being the On-Call supervisor. This will occur for 7 days at a time and you must be available 24 hours a day for those 7 days to respond to emergencies. This could include but not limited to the Team Leader going into a location to work, transport a resident to emergency services, answering On-Call phone questions, giving directions and solutions to situations that may arise.

Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience:

Experience in working with individuals with developmental disabilities and supervisory preferred but not required. On the job training and professional classes taken through DDA can be provided to the right candidate.

Language Ability:
Ability to effectively present information and respond to questions from groups of managers, clients, customers, guardians and the general public.

Math Ability:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Complete monthly bank reconciliations.

Reasoning Ability:
Ability to define problems, problem solve, collect data, establish facts, and draw valid conclusions.

Computer Skills:
To perform this job successfully, an individual should have knowledge of Word Processing software and Spreadsheet software.

Certificates and Licenses:

Can successfully pass a background check on an annual basis. Driver’s license, vehicle and current auto insurance upon hire. Communitas provides and pays for the required training ex., but not limited to a Food Handler’s Permit, Blood Borne Pathogens Training, Nurse Delegation Training, CPR/First Aid, NAR 40 and 75 hour cert following RST training.

Supervisory Responsibilities:

Directly supervises a case load, of no more than 6 individuals and their care teams that involves Direct Support Professionals. Provide daily objectives to oversee the day to day operations if their team(s). Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; communicate any issues with upper management; implanting strategies that team members can use to achieve a goal; addressing complaints and resolving problems. Will provide feedback to the Case Manager who will then conduct the annual reviews of the Direct Support Professionals on their team(s).
