Position: Nahant Street Coordinator Location: Wakefield, MA Pay rate: $20 per hour Hours: Full-time, Monday - Friday (12pm-8pm) Must be flexible to meet needs of Individuals Benefits of Working for us!
Pay increases after 1 year and 3 years of service, pending performance evaluations

Summary & Responsibilities The Nahant Street Coordinator is responsible for day-to-day oversight of the residence, including implementing agency policy and practice and ensuring compliance with the regulatory requirements of the DDS. Ensures that the home environment is maintained to the highest standard. Protects the rights, freedoms and confidentiality and supervise the medical, nutritional health and general welfare of individuals within the program.
Provide oversight of the medical health, nutritional standards, educational and general welfare of the individuals.
Completes all reports and documents required by the DDS pertaining to the individuals in the program.
Ensures that the program is maintained in a safe, sanitary, attractive, and comfortable condition.
Protects the integrity and privacy of the program. Protect each individual's rights and freedom and maintain confidentiality of information relating to individuals in the residential program.
Adheres to and observes all State guidelines and (DDS) regulations regarding the operation of the program, including staffing requirements and ratios.
Participates in the development of the (ISP) Individual Service Plan and periodic review, in collaboration with all staff.
Holds regular meetings with individuals to discuss issues, activity plans, and household routines.
Engages with the individuals and participate in program and community activities. Responsible for community outreach to ensure integration and relationship building.
Provide positive behavioral supports to the individuals and assist in managing behavioral challenges.
Some driving required
Completion of weekly and monthly reports as required
Attends all mandatory agency meetings and trainings.
Demonstrates and implements knowledge of Communitas policies and standards as well as funding source regulations.
Follows HIPPA, security regulations and/or guidelines and maintain confidentiality at all times.
Provide Direct Support Coverage as needed.
Other duties and special projects as assigned.
Qualifications
High School diploma required (BA degree in Human Services or related field preferred)
At least 3 years of experience supporting adults with developmental disabilities or similar needs required
Experience working with families of individuals supported and other service providers preferred
Must hold current CPR & First Aid certification
A passion and dedication to supporting our people is a must
