POSITION DESCRIPTION:

Responsible for providing psychosocial services under the general direction of the Behavioral Health (BH) Program Coordinator in accordance with policies and protocols established by CommuniHealth Services (CHS) and other regulatory agencies. The provider is responsible for implementing age appropriate educational programs, assessing psychosocial factor causing emotional and behavioral distress, determining appropriate methods of treatment to address these factors, and advocate for patient to motivate toward obtaining patient-determined goals of treatment. The provider implements interventions, methodologies, and functions in such areas as individual and group counseling, protective services and family consultations. This position requires compliance with CHS written standards, including the Compliance Plan and policies and procedures (including the Code of Conduct), as well as assistance in the continuous monitoring, reporting and corrective action processes of corporate compliance.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Assessment of psychosocial and behavioral health needs, assess motivation toward change, assess challenges toward treatment success, treatment planning, and implementation of psychosocial services for patient while working to coordinate across multiple settings.

2. Work with the patient’s health team members to provide needs assessment including: Gathering and assessing information regarding the patient’s needs, mental status, support systems, and inform patient of available financial, community, and governmental resources. Work with patient’s family members as deemed appropriate in regard to confidentiality.

3. Responsible for the content of education and counseling programs that will provide maximum benefit for each patient, both on an individual and group basis.

4. Provides on-site evaluation of patients referred by other members of the health center, provide brief interventions in an acute crisis situation and coordinate referrals for any in-patient psychiatric needs.

5. Counsel patients and interpret any specific psychosocial factors that are correlated to current behavioral health conditions, and advising on the incorporation of skills for the alleviation of anxieties and/or fears.

6. Interpret nature of mental illness and prescribed course of treatment to patients and families. Appropriately communicates family issues to other staff members.

7. Educates patient on patient’s partnership role in treatment including improvements to self-care practices, medication compliance, and compliance of treatment plan to assist with prevention of unnecessary ED or hospitalizations.

8. Provide short-term counseling that is goal-oriented to meet specific behavioral health and psychosocial needs and explore available options. This may include identifying dependency issues, identity crisis, interpersonal communication barriers, negative relationship interactions, and a need to improve boundaries, etc. involving family as necessary.

9. Ensures confidentiality of interviews, treatment and documentation.

10. Work closely with all members of the health care team to develop a comprehensive treatment plan to meet the patient’s identified needs, revise treatment plan as needed, and drive the plan to insure optimal treatment benefit.

11. Conduct surveys and studies relating to psychosocial status and patterns of population groups as directed.

12. Prepare psychosocial educational materials as needed or required and conduct behavioral health related educational presentations to clinical staff, patients, and patient families.

13. Coordinates with the medical staff in an integrated health team approach for the provision of services to patients. Provide referrals for additional services as needed.

14. Completes required CHS training per policy, maintains compliance with and knowledge of all CHS departmental, facility specific and system-wide policy and procedure, attends mandatory departmental meetings and/or trainings, and follows CHS Mission, Vision and Values in the delivery of all services.

15. In addition to the outlined duties above, as a patient-centered medical home, the incumbent fulfills the principles and theories outlined in organizational policies including assisting navigating patients as needed through the health system, making appropriate referrals to case/care management, communicating with the healthcare team to address patient/family/caregiver specific needs, etc.

16. Maintains a working knowledge of all health information management issues such as HIPAA, Corporate Compliance, and other such healthcare regulations.

17. This position may require work beyond normal working hours

18. Performs other duties as may be assigned.

EDUCATION/LICENSURE:

1. Master’s Degree in Social Work or Counseling

2. Nationally Certified for Applicable Degree

3. Licensed in the State of Louisiana as a Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marrige and Family Therapist or other such licensed professional to providing counseling services

4. Must possess current CPR certification

SKILLS/EXPERIENCE:

1. Must be able to read, write, and speak the English language in an understandable manner.

2. Knowledge of assessing, planning, and implementation of psychosocial services.

3. Ability to counsel/educate patients and interpret any specific psychosocial needs or psychosocial disorders to professionals and lay groups. Ability to conduct surveys and studies relating to psychosocial status and patterns of population groups as directed.

4. Ability to review and evaluate recent psychosocial health research and interpret it to professionals and patients.

5. Ability to perform short-term counseling and crisis intervention as necessary.

6. Ability to assist in the implementation of social programs.

7. Ability to coordinate with clinical staff in the provision of services to patients. Provide referrals for services as needed.

8. Ability to provide in-service presentations to staff as needed or required.

9. Ability to provide community service information as an outreach worker if required.

10. Ability to conduct behavioral health related educational presentations to clinical staff, patients, and patient families.

11. Good computer skills
