POSITION TITLE: Health Coach

DEPARTMENT: CPSP

REPORTS TO: Sexual and Reproductive Health Services Manager

About CommuniCare OLE

Established in 2023, CommuniCare OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY:
The Health Coach works as part of a multidisciplinary team to provide clinical health education and quality improvement services to patients at CommuniCare OLE. CommuniCare OLE is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health, health education and outreach services to the low income, multi-ethnic, uninsured and underinsured populations.


EDUCATION, EXPERIENCE, TRAINING

  • Medical Assistant (MA) certification and one-year professional experience as a certified Medical Assistant OR Licensed Vocational Nurse (LVN) OR BA/BS degree in health and human service field and at least one year of experience working in a health clinic, counseling agency, or other closely related social service agency. Experience providing clinical care services preferred
  • Passion for sexual and reproductive health services and serving adolescents required.
  • Bilingual Spanish/English or Russian/English preferred.
  • Valid Driver’s license and proof of insurance required.


KNOWLEDGE OF WORK

1.

Acquire certifications in sexual and reproductive health counseling, minor consent and confidentiality, assessment of health education and psychosocial risk status, and appropriate use of counseling/referral protocols.

2.

Responsible for providing birth control method education, sexually transmitted infection education, pregnancy options counseling, PrEP and PEP information, case management of PrEP patients, healthy relationship assistance, and related topics during patient visits.

3.

Independently run Safe Care Clinic schedule for sexual and reproductive health education and related clinical services.

4.

Use counseling and Motivational Interviewing techniques to actively engage patients and facilitate implementation of customized care plans and healthy behavior change.


DUTIES AND RESPONSIBILITIES

1.

Communicates effectively with patients, medical staff and outside medical organizations to assist clients with access to community resources, facilitation of medical care, scheduling appointments, obtaining medical records and problem solving as needed.

2.

Provide clinical sexual and reproductive health education and limited outreach services.

3.

Provide initial patient intake during clinical visits including medical history, financial screening, eligibility form, vital signs, and medical intake form. Collect patient vitals including height measurement, weight, and blood pressure data. Complete accurate medical chart documentation.

4.

Perform specimen collection and point of care testing including but not limited to urine pregnancy testing, urine STI testing, and rapid HIV and syphilis testing.

5.

Provide birth control injections and other injections as trained to administer.

6.

Assist in the collection and compilation of sexual and reproductive health data.

7.

Facilitate, co-facilitate, or arrange presentations or other educational activities related to sexual and reproductive health.

8.

Support Medical Assistants (MA) Only Schedule with education and clinical support.

9.

Support Mobile Medicine team during assigned shifts for sexual and reproductive health education and related clinical services.

10.

Support MAB (medication abortion) clinic during assigned shifts at Vida Family Health Center.

11.

Collaborate with Registered Nurses (RN) on incoming non-face to face care (Telephone Encounter), including: (1) Perform call backs regarding lab results. (2) Complete Confidential Morbidity Reports, Newborn abstractions, and Emergency Room abstractions

12.

Under medical staff supervision, prepare patients for RN visits; including and not limited to preparing the room, measuring vital signs, clarifying the reason for the visit, performing INR and A1c testing.

13.

Develop health education materials such as social media content, flyers and handouts

14.

Attend on-going training and certification, staff meetings, and clinical site meetings.

15.

Assist in providing staff training on preventative sexual and reproductive health care and quality improvement.

16.

Support the Chief Quality Officer in quality assurance and quality improvement as assigned.

17.

Performs other duties as assigned
