Position Overview:

We are seeking a dynamic and compassionate External Client Care Manager to join our team. The External Care Coordinator (ECC) will play a pivotal role in assisting clients in determining the type and duration of services needed to meet their needs, as well as providing continuous reassessment of their plan of care. This position involves traveling to clients in Yolo and Solano Counties (W. Sacramento - Benicia), with a company vehicle provided. The ECC is responsible for assisting with sales and marketing initiatives for your territory. This position is responsible for making and maintaining sales relationships with care partners as well as assisting in the creation and implementation of sales plans in alignment with your location’s target goals. Participation in and organization of community or networking events is a vital part of this position’s marketing efforts.

Responsibilities:

• Assist clients in determining the type and duration of services needed to meet their needs
• Provide continuity of care for clients during and after referral
• Facilitate the provision of in-home care and support services to clients
• Assist with sales and marketing initiatives for the territory
• Make and maintain sales relationships with care partners
• Create and implement sales plans in alignment with target goals
• Participate in and organize community or networking events to support marketing efforts

Qualifications

• Minimum 1 year of experience at a HOME CARE ORGANIZATION
• Previous experience in client care coordination or related field preferred
• Demonstrated history of 80% close rate
• Excellent interpersonal and communication skills
• Strong organizational and time management abilities
• Ability to work independently and as part of a team
• Proficiency in Google Workspace, WellSky and other relevant software
• Valid driver's license, auto insurance and clean driving record
• HCA # clearance and ability to pass DOJ/FBI livescan clearance

Job Type: Full-time

Pay: $35.00 - $38.00 per hour

Expected hours: 40 per week
