HOSPICE SOCIAL WORKER
Comfort 1 Hospice is seeking a Social Worker to join our dedicated Hospice Team in South Bend, Indiana. As a member of the Comfort 1 Hospice interdisciplinary team, you will provide psycho-social assessments, social service counseling, needs assessments, and guidance to patients and family members. Additionally, the Social Worker will assist the Physician and other members of the Hospice Team in understanding significant social/mental stress and/or disorder that increases the symptoms related to terminal illness. Your visits will be between skilled nursing, independent and assisted living facilities and private residences.
