Bloom Home Care Supervisor (non-medical)

We are currently accepting applications for a quality Home Care Supervisor able to work :

• Full-Time / Part-Time
• Monday to Friday daytime hours with occasional evenings and weekends

Home Care Supervisor

• Works under the direct supervision of the Bloom at Home Manager.
• Provides leadership and guidance for Personal Care Provider (PCP) staff.
• Coordinates admission and scheduling of new clients to Bloom at Home services
• Schedules client visits according to client request and policy guidelines
• Informs client and personnel of scheduled visits and duties
• Maintains client care plans and records
• Makes periodic supervisory visits to clients being cared for by personal care providers
• Ensures compliance with electronic visit verification
• Covers open shifts and on-call responsibilities as needed to ensure optimal client service
• Tracks new referrals
• Maintains high-quality communication with clients, employees, and other providers for coordination of care
• Marketing and outreach activities
• All other duties as assigned

Home Care Supervisor Requirements :

• High school diploma or general education degree (GED).
• One year of related experience.

COMPENSATION

Starting wage $18.00- $22.00 / hour DOE.
