COLORADO COALITION FOR THE HOMELESS is Hiring a Behavioral Health Clinician - 1680 Near Denver, CO

The Colorado Coalition for the Homeless seeks a Behavioral Health Clinician to work collaboratively with the multi-disciplinary care teams throughout CCH to provide assessments and related diagnoses, brief therapeutic interventions, and works directly with other care providers to complete comprehensive assessments to determine appropriate housing placement. This position is located at 2100 Broadway St, Denver Colorado 80205 and reports to the Clinical Program Manager. 

Behavioral Health Clinician II - LCSW/LPC: $70,176 - $80,702/Salary - Fully Licensed candidates are eligible for a $5,000 sign-on bonus (prorated based on weekly schedule).

Behavioral Health Clinician I - LSW/LPCC: $29.77 - $34.23/Hourly

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions 
  • Works in close collaboration with street outreach workers to identify and outreach adults experiencing homelessness who need behavioral health care. 
  • Performs intensive outreach and engagement services to invite adults to engage with recovery supports through referrals to behavioral healthcare, primary care, case management, peer support, housing, and related services.
  • Provides targeted brief interventions and therapy, which is often crisis and recovery focused and episodic in nature.
  • Provides individual and group therapy for adults who are interested in ongoing behavioral health care, using treatment models including motivational interviewing, solution focused therapy, harm reduction, and cognitive behavioral therapy.
  • Completes biopsychosocial assessments and utilizes DSM-V-TR criteria for diagnosis. 
  • Develops comprehensive treatment plans, monitoring patient progress and accessing direct psychiatry and medical care through Stout Street Health Center
  • Assists in connecting adults experiencing homelessness with the resources necessary to obtain and maintain housing, healthcare, community connection and meaningful daily activity.
  • Collaborates with the consulting psychiatric prescribers and primary care physicians as part of the integrated health team.
  • Provides proactive collaborative services to clients/patients with face-to-face and telephone contacts, or as otherwise appropriate.
  • Develops and maintains a partnering relationship with SSHC patients engaged through outreach in order to assist in the implementation of the treatment plan.
  • Provides transportation in CCH vehicle or arranges for other transportation assistance to help clients in accessing care and meeting treatment goals.
  • Serves as liaison with collaborating agencies to establish and maintain effective working relationships with organizations and groups providing services to shared clients (care coordination).
  • Advocates for the needs and rights of individual clients and, when indicated, for systemic change both within and outside CCH for the benefit of PATH and Outreach clients.
  • Performs the initial intake in electronic health record (EHR) for clients/patients who want to connect with psychiatric care at Stout Street Health Center and completes additional paperwork as required.
  • Maintains compliance with all CCH and grant-specific reporting requirements.
  • Demonstrates respect, cultural competence and knowledge of trauma informed care in interactions with patients, Coalition staff, and other community providers.
  • Maintains a work environment that is marked by respect for others; that values inclusiveness and builds workforce diversity; and that fosters cooperation and teamwork
  • Record all clinical services in a timely and complete fashion in the EHR.
  • Maintain up to date records in EHR.
  • Record telephone and other clinical communications as needed in the EHR.
  • Manage tasks, PAQ and scanning as needed to have a complete and current chart.
  • Maintain accurate information regarding client’s insurance status and complete CCARs/DACODs and other clinical authorization documentation as needed.
  • Utilize the EHR and any future upgrades as directed.
  • Maintaining a passing score in all compliance audits 
  • Maintains the department’s monthly productivity and timeliness standards.
  • Participates in peer review and peer clinical audits as requested.
  • Attends meetings on time and participates in clinical and administrative meetings as deemed appropriate and necessary for the job.
  • Participates in regular clinical and nonclinical supervision.
  • Remains available by cell phone during all assigned shifts.
  • Returns all voicemails and e-mails in a timely manner.
  • Performs other duties as assigned.
Qualifications Summary 
  • Master’s degree in social work, counseling or similar field.
  • Licensed as a LCSW or LPC preferred, with a CAC II or higher strongly preferred. If not licensed, must already be registered with DORA as a candidate for licensure (i.e. LSW,LPCC) and within 6 months of obtaining LCSW or LPC license.
  • Experience interacting with patients in integrated health care, mental health care, or substance use treatment settings preferred.
