Job Location: Codman Square Health Center - Dorchester, MA
Salary Range: $19.98 - $31.13 Hourly

Description

Position Summary:

Reporting to the Program Manager for Substance User Health Services, the Clinical Coordinator is responsible supporting the clinical work of the substance use team. This position has a critical role on the team by helping to coordinate the care received by patients enrolled in the Substance Use program by being the primary point person for both patients and SUD clinicians. The position also works closely with the SUD nurse during delivery of care in the clinic.

Primary Responsibilities:

  1. Patient Support and Engagement:
    • Coordinate the intake process of individuals seeking services within the Substance Use Program, either through internal or external referrals
    • Assist patients throughout the health center by supporting patients seeking services, e.g., detox or further treatment
    • Ensure patients receive medications to treat substance use disorder in a timely way through refills, prior authorizations, and pharmacy coordination
    • Work closely with the nurse care manager during in person care days, supporting the timely movement of patients through the clinic
    • Establish and maintain a compassionate and supportive rapport with patients, ensuring their effective engagement
    • Coordinate groups as directed by clinical professionals.
  1. Administrative Support:
    • Maintain accurate and confidential patient records in compliance with established protocols and regulations.
    • Maintain updated and accurate patient lists
    • Assist in scheduling patient appointments, follow-ups, and referrals, ensuring efficient coordination of care.
    • Collaborate with the administrative team to ensure smooth operations and facilitate communication between clinical staff and patients.
  1. Resource Coordination:
    • Assist patients in accessing community resources, such as housing assistance, job placement, legal aid, and other relevant support services.
    • Collaborate with external organizations and agencies to facilitate referrals and support networks for patients
  1. Team Collaboration:
    • Act as a liaison between clinical professionals, administrative staff, and patients, fostering effective communication and coordination within the program.
    • Participate in team meetings, contributing insights and updates on patient progress, needs, and challenges.
  1. Adherence to Policies and Regulations:
    • Adhere strictly to confidentiality guidelines and ethical standards in handling patient information.
    • Remain updated with relevant policies, procedures, and regulations related to substance use treatment and healthcare practices.

Qualifications


Skills and Qualifications:

  • Experience as a Medical Assistant preferred or ability to learn clinical support skills
  • Experience working in a direct service organization, including working with clinical staff.
  • Highly motivated, organized individual who is able to manage workload independently
  • Be able practice trauma informed care with patients with Substance Use Disorder
  • Enjoys challenges and working with a diverse workforce
  • Experience with data management and analysis including managing patients on spreadsheets or other digital tools
  • Experience using an Electronic Medical Record
  • Commitment to substance user health and improving social determinants of health
