Fee For Service Occupational Therapist

Your Choice At Home is an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA). Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services.

As the Occupational Therapist, you will be responsible for evaluating the clients' level of physical functioning in the home and for working with the client, family and/or others for the planning, provision and coordination of occupational therapy services as ordered per MD prescription. The Occupational Therapist also works with other professional staff to establish rehabilitation and maintenance goals and activities.

Responsibilities include:

  • Developing a plan of treatment which encompasses treatment modalities, frequency of visits, expected outcomes and short-term and long-term goals.
  • Assessing patients and developing a therapeutic plan of care for therapy including activities of daily living.
  • Evaluating home environment and recommending adjustments to facilitate patient safe functioning in the home and to promote conservation of patient's energy.
  • Evaluating patient's ability to perform activities of daily living in collaboration with agency professional personnel and recommends modalities to maximize patient's functional activities.
  • Evaluating and making recommendations for the use of medical equipment and other adaptive tools for ADL.
  • Considering the patient's psychological response to the effects of illness and subsequent physical changes.
  • Addressing patients' needs in a treatment plan with realistic short-term and long -term goals.
  • Documenting frequency and duration on evaluation.
  • Implementing the plan of treatment under physician direction and in collaboration with agency professional personnel.
  • Applying occupational therapy procedures to achieve identified goals
  • Instructing and monitoring patients, families, and health team members in specific functional exercise programs to increase strength, range of motion and coordination of upper extremities.
  • Teaching use of adaptive equipment to client, family, and other appropriate health team personnel to achieve maximum independence.
  • Instructing patients, family members, and other appropriate health team personnel in treatment program exercises for continuation of a maintenance program.
  • Counseling patient/family to aid in interaction and adjustment to patient's functional limitations.
  • Observing, recording, and reporting to the physician and program personnel, the patient's reaction to treatment and any changes.
  • Assisting in developing alternate plan options for medically and/or financially ineligible referrals.
  • Maintaining communication with other persons providing care and provides information to these persons to further aid in the rehabilitative process.
  • Modifying the plan of treatment as needed and is able to reflect these changes within the written plan of care and progress notes and verbalize the modifications to the CHHA RN.

What you offer:

  • License and current registration to practice as an Occupational Therapist in the State of New York.
  • Registered with the American Occupational Therapy Association, or, is a graduate of a program in Occupational Therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American occupational Therapy Association
  • A minimum of 1 year experience in a hospital, long-term care, rehab facility or home health agency.
  • Experience working with a pediatric population is required. A combination of experience working with pediatric, adult, and geriatric populations is a plus.
  • Demonstrates knowledge of Medicare/Medicaid, regulatory and credentialing criteria and standard.
  • Must be able to make home visits in potentially unsafe and unsanitary home environments and be comfortable with travel on public transportation in potentially unsafe areas of the community. Standing, sitting, walking long distances, bending, reaching, stretching, and climbing stairs may be required. Lifting up to 50 pounds unassisted and up to 325 pounds utilizing an assistive device in the home environment may also be required.
