Responsibilities:

• Patient case and care management which includes following up on pending orders and providing updates
• Review and audit patient chart notes and records for compliance
• The incumbent may have access to highly confidential patient, employee, and proprietary information, and must handle and protect the information in accordance with CMS policies, HIPAA requirements, and the highest level of ethical standards
• Supports other team members in their duties during peak times or during absenteeism
• Maintains all practice-based policies and procedures in accordance with CMS, federal and state regulations, accrediting bodies and hospital/SNF policies
• Able to receive constructive feedback for problem resolution
• Ensures a safe work environment, adequate supplies (office and clinical) and appropriate resource utilization for the practice
• Maintains collaborative relationships with other clinical and non-clinical staff and leaders
• Ensures that all patient concerns, account grievances and issues are documented and reported
• Maintains and sets expectation for professionalism, confidentiality, and high degree of work ethic and integrity
• Ensures an integrated and coordinated approach to a successful and patient-centered practice through internal and external collegial relationships and partnerships

Experience:

• Have at least one year of patient care and case management experience preferred.
• Demonstrates ability to conduct and interpret quantitative and qualitative analysis.
• Must have excellent interpersonal, verbal, and written communication skills.
• Must have attention to detail and problem solving skills.
• Have the ability to prioritize multiple agendas, issues and concerns, including the ability to follow through on commitments and issues in an accurate and timely manner is required.
• Must have knowledge of US clinical healthcare systems.

Job Type: Part-time

Pay: $30.00 per hour

Expected hours: 20 – 25 per week
