CSO is looking for anOutreach Specialist to join our Community Partner Program!
Be part of our team ofcaring and skilled professionals as an Outreach Specialist in theCommunity Partner Program at Clinical & Support Options. We are amultidisciplinary team of Community Health Workers, Clinicians, and Nursesproviding Care Coordination and support to adults with complex medical,behavioral health and substance use challenges. Through face-to-faceoutreach and telephonic care coordination, our team supports each person withtheir individual wellness goals in connection with their overall health. At CSO, we use a trauma informed and person- centered approach.
CSO offers Training in: Traumainformed care, Motivational Interviewing, Cultural Diversity, Equity andInclusion, Mindfulness Skills and more.
Responsibilities
Qualifications
Associate's or Bachelor's degree (Psychology, Sociology, Social Work, etc.) from an accredited college or university required (Bachelor's preferred.) Good listening skills, nonjudgmental ability to foster self-sufficiency and an ability to establish trusting relationships through compassion, empathy and insight. 1-2 years of experience is preferred. Capacity to be an effective member of a team. Demonstrated sensitivity to the needs and perspectives of people from diverse cultural or linguistic backgrounds. Computer proficiency required including ability to work with email, electronic health record and word processing software. MA driver's license and reliable transportation required.
