Job Summary:

Provide intensive case management services to a population of adolescent females in a residential setting.
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have experience in crisis management, and behavioral health. As a Case Manager, you will be responsible for providing support and guidance to clients in need, helping them navigate various social services and resources.

Responsibilities:
- Provides intensive case management service within a team setting to a population of adolescent females residing in residential treatment on their journey of wholeness and healing.
-Provide outreach, advocacy and support services to clients at Clearview.
- Assist clients to identify strengths, needs and goals in residential, health, vocational, educational, social and economic domains.
- Coordinate necessary specialized services including psychiatric, psychological, medical, educational and daily living needs and desires of the client.
- Facilitate client access and utilization of natural community resources.
- Acts as an advocate on behalf of the client and ensures the protection of rights through direct intervention, education, and facilitation of advocacy groups in the community.

Experience:
- Must have a valid driver's license, minimum one year of full time experience serving people on their journey to mental wellness. Other education and experience may be considered, such as a combination of relevant experience and/or education equaling five years.

Pay: $22.00 - $26.00 per hour
