Registered Nurse Job Description (Home Health)

JOB SUMMARY:
A qualified Registered Nurse at Clear Care performs the initial client evaluation,
develops the plan of care and performs supervisory visits as required by state.
This position may assume the responsibilities of the RN Manager, if applicable.
QUALIFICATIONS:
1. Graduate of a state approved school of nursing and currently licensed in the state of Georgia.
2. Maintains CPR Certification.
3. Minimum of one (1) year experience in Home Care/Personal Care, preferred.
4. Acceptance of philosophy and goals of Agency.
5. Ability to exercise initiative and independent judgment.
RESPONSIBILITIES:
1. Provides the following core services
a. Care Management
b. Nursing Assessments
c. Teaching and Training of Disease Processes
d. Nursing Delegation
2. Makes the initial evaluation visit and regularly reevaluates the client’s needs.
3. Initiates the plan of care and necessary revisions.
4. Provides supervisory visits as required by the State of Georgia.
5. Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client
6. education and training.
7. Informs client and/or client representative of changes in the condition and needs of the client.
8. Understands and adheres to established policies and procedures.
9. Teaches and counsels the client and family/significant others regarding their service needs and
other related problems of the client at home when applicable.
10. Maintains accurate and complete records
11. Attends staff meetings, client care conferences and in-services as scheduled.
12. Supervises and teaches other personnel, when applicable.
13. All other duties as assigned.

1. Supervised by: Director of Clinical Services
2. Workers Supervised: LPN, CNA, HHA

Job Types: Contract, PRN, Per diem

Pay: $38.13 - $41.59 per hour

Weekly schedule:

• Monday to Friday
• Weekends as needed

Experience:

• Home care: 1 year (Preferred)
• Hospital experience: 1 year (Preferred)
• ICU experience: 1 year (Preferred)

License/Certification:

• CPR Certification (Required)
• RN License (Required)

Ability to Relocate:

• Smyrna, GA: Relocate before starting work (Required)
