Description

The Case Manager – Permanent Supportive Housing is responsible for providing an array of supportive services to formerly chronically homeless men and women who are enrolled in the program in an effort to promote long-term stability, self-sufficiency, and eventual movement into mainstream housing.

  • Provide information and referrals to callers or walk-ins seeking housing assistance, as assigned
  • Assess participants’ eligibility and program appropriateness; obtain required documentation for program eligibility
  • Assist participants to secure appropriate housing based on composition, geographic preference, and other participant determinants
  • Assist with the rental application process; help participants make informed decisions
  • Seek to assist participants to maintain long-term housing
  • Complete accurate and timely Housing Quality Standards inspections
  • Assist participants to understand the landlord/tenant relationship and become good leaseholders
  • Assist participants to understand their fair housing rights
  • Maintain current information about consumers’ benefits (basic income and medical information) to insure the subsidy amount is accurate with regard to income
  • Assist participants to seek and secure mainstream benefits for which they are eligible
  • Assist participants to meet physical health and behavioral health needs through referral to in-house services and external resources
  • Assist participants to seek employment, disability benefits, and/or education as necessary
  • Prepare and submit check requests for monthly rent/utility assistance in a timely manner
  • Provide outreach to property owners and participants to market the Permanent Supportive Housing program
  • Develop and maintain good working relationships with property managers
  • Assume responsibility for data management and reporting; enter complete and accurate data into HMIS
  • Maintain complete and accurate participant charts with current case notes and financial documentation
  • Exhibit and maintain up-to-date knowledge of community resources; make referrals to the appropriate community resources
  • Provide services through face to face and phone consultations; conduct home visits at least once a month
  • Obtain household start-up items for new program enrollees
  • Meet with participants based on need, length of time in program, etc. – a minimum of once per month face-to-face contact is required for each participant for the duration of participation
  • Develop individualized service plans (ISP) with each program participant based on the identification of needs, client personal goals/aspirations, and the outcome goals and objectives of the program
  • Facilitate community and internal service referrals and conduct follow-up services for referral outcomes
  • Provide direct assistance (e.g., completing applications, accompanying the client to appointments, etc. (in the pursuit of entitlement benefits, healthcare needs, mental health services, etc.
  • Maintain accurate and timely client hard copy and digital file information
  • Facilitate and/or identify external entities that can facilitate, small and medium groups on life skills, employment, physical and mental health, substance abuse and other related subjects.
  • Prepare and submit reports to supervisor as requested 
  • Promote the program, attend community based organizational meetings, and trainings, when and where applicable. May be required to work some nights and weekends
  • Perform other duties as assigned

Qualifications


EDUCATION AND EXPERIENCE

  • Undergraduate degree in social work or a closely related field
  • 2 years of recent housing experience with local, state, and/or federal programs preferred
  • 2 years of experience working with persons with mental health and/or substance abuse challenges
  • Outstanding organizational, filing, verbal, and written communication skills
  • Strong interpersonal skills and ability to work with diverse community groups and shelters
  • Computer literate and proficient in Microsoft Office software\

REQUIRED SKILLS/ABILIITES

  • Display professional appearance in accordance with agency policy
  • Handle difficult situations in a discreet manner.
  • Maintain attendance within limits of agency guidelines
  • Maintain confidentiality regarding client information.
  • Attend agency and staff meetings, and in-service trainings, as scheduled.

PHYSICAL REQUIREMENTS

  • Remaining in a stationary position, often standing, or sitting for prolonged periods
  • Light work that includes moving objects up to twenty pounds
