BRIEF DESCRIPTION:
The purpose of this position is to provide outreach, screening, counseling, intake and support services for clients. This is accomplished in a clinic or outreach setting by screening for services, taking medical histories and related information, assessing needs, providing education services, administering medications or immunizations, drawing labs, investigating information, and maintaining records. Also by completing eligibility and notification services, outreach and home visit activities, testing, identifying risks, disbursing education and maintenance materials, providing medications, and completing and updating charts and reports. Other duties may include screening, counseling, intake and support services for clients. This is accomplished in a clinic or outreach setting by screening for services, taking medical histories and related information, assessing needs, providing education services, administering medications or immunizations, drawing labs, investigating information, and maintaining records.. Other duties may also include participating in health or medical information educational fairs and clinics, participating on committees, preparing for clinics, and referring patients to ancillary providers.

OTHER REQUIREMENTS:
Medical Assistant Certification, CPR Certificate, First Aid, must meet immunization requirements after employment begins, and have the ability to wear and use the N95 respirator to perform essential functions of the position.

# Code Essential Functions
1 L Regular and reliable attendance in compliance with given schedules is a mandatory requirement of this position. Schedules may be subject to change with or without advance notice
2 S Screens and triages clients for services and eligibility in a clinical or outreach setting by verifying eligibility, obtaining vital signs such as blood pressure, height, weight and temperature, discussing and recording current medications and medical histories, and determining needs for lab and clinic services.
3 S Provides educational materials and counseling on communicable disease, diet and nutrition, prenatal care, breastfeeding, nutrition, medication, and general health care by developing and presenting materials, distributing materials and related information, setting up equipment and materials for classes, soliciting promotional items, and attending and participating in training classes and local events.
4 S Assists in preparing clinic, assisting with exams, treatment procedures, required reporting, and obtaining additional outside information such as agency or physician referrals. May also provide necessary testing, provide or administer medications or immunizations, perform venous blood draws, obtain lab results, provide counseling, education and support services for clients, complete initial screenings, identify risks and refer patients to ancillary providers.
5 S Completes administrative tasks by ordering supplies, maintaining inventory, stocking materials, filing reports on communicable diseases and immunizations, entering data and statistical information, maintaining demographics, sending vaccination reminders, researching files and records, locating charts, and preparing billings.
6 S Completes administrative tasks by answering the phone and processing inquiries, answering program questions, providing eligibility paperwork, and processing mail.
JOB REQUIREMENTS:
-Description of Minimum Job Requirements-
Formal Education Work requires knowledge of a specific vocational, administrative, or technical nature that may be obtained with six months/one year of advanced study or training past the high school equivalency. Junior college, vocational, business, and technical or correspondence schools are likely sources. Appropriate certification may be awarded upon satisfactory completion of advanced study or training.
