I. Summary of Position

The Chronic Disease Prevention Coordinator is responsible for developing and managing a diverse portfolio of projects, including programmatic initiatives, policy change, and collaborative efforts that facilitate healthy lifestyles and reduce chronic disease rates among New Orleans residents. The Coordinator will work to ensure that the New Orleans Health Department has clear, evidence-based strategies aimed at reducing the prevalence of chronic disease through its programs, policy initiatives, partnerships, and community engagement.

II. Organizational Relationships

Supervisor: Jeanie Donovan, Director of Population Health & Disease Prevention

Assignments received from: Director and Deputy Director of the New Orleans Health Department

Interacts with:

  • Public and private sector organizations, including health care providers, nonprofit organizations and universities.
  • NOHD program managers and initiative coordinators
  • Non-profit organizations and organization leaders
  • Collaborative coalition leaders
  • The Office of Neighborhood Engagement
  • Chief Administrative Office
  • Louisiana Department of Health

Individuals supervised: Interns as needed

III. Duties and Responsibilities

A. Core Functions (provide at least 3):

  • Conduct and disseminate research and analysis related to chronic disease prevalence among the residents of New Orleans by race, gender, ethnicity, geography, socioeconomic status and other relevant factors.
  • Actively investigate emerging and evidence-based programs and policies that address chronic disease and the factors that impact chronic disease outcomes.
  • Work collaboratively with NOHD staff and organizational partners to develop, plan, and execute chronic disease-focused policy and programmatic initiatives.
  • Identify, pursue, and manage external funding sources that support the implementation of chronic disease focused public health interventions.
  • Effectively communicate and promote chronic disease focused programs and policy changes to stakeholders and target populations.
  • Assist in the measurement and evaluation of NOHD’s chronic disease policy and program interventions.
  • Represent NOHD at external meetings and working groups as needed.

B. Additional Responsibilities (if applicable):

  • Engage in grant writing and administration; track program budgets
  • Contribute to equity, quality improvement and organizational development initiatives within the Health Department
  • Participate in the Health Department’s emergency response activities as needed.
  • Collaborate with other City Hall departments on employee wellness activities related to health promotion and chronic disease prevention
  • Develop creative materials, including white papers, public reports, social media posts, one pagers and other materials as needed.
  • Special projects as assigned

C. Physical Requirements, if any:

  • Occasionally required to lift 25 pound boxes

IV. Examples of Work (provide 3-5 examples)

  • Data Analysis: Leverage local, state, and federal data sources to develop an analysis and short report that details recent trends and patterns related to the prevalence of diabetes, heart disease, and obesity among New Orleans residents.
  • Programs: Work with NOHD’s Healthy Start Initiative and local health care providers to implement a chronic disease management program that works directly with Healthy Start participants that are at high risk for or have an existing chronic disease.
  • Policy: Research and develop a proposal related to the restricting the density, type, and location of tobacco retailers via local zoning and licensing regulations.
  • Grant Writing: Identify and apply for public and private funding opportunities that support NOHD’s efforts to implement chronic disease focused programs or policies.
  • Partnerships: Partner, sponsor and participate in community-based collaboratives and initiatives aimed at decreasing chronic disease rates and facilitating healthy lifestyles in Orleans parish, such as the New Orleans Food Policy Advisory Committee and the Region 1 Louisiana Health Communities Coalition.

V. Qualifications and Skills

A. Minimum Qualifications and Skills Required by Department:

  • A Bachelor's Degree from an accredited college or university in public health, social work, science, a social science, or a closely related field.
  • Two (2) years of professional experience in the planning and implementation of public health programs and/or policies.
  • Proficiency in Microsoft Word, Excel, PowerPoint, graphic design software, social media platforms, email marketing platforms
  • Strong interpersonal skills

  • Strong conceptual, strategic and visionary skills

B. Other Qualifications, Skills, and Competencies:

  • Knowledge of and experience in effective public speaking techniques and the ability to present complex information in an effective and concise manner.
  • Demonstrated experience and knowledge of disparities, health equity, and social determinants of health.
  • Demonstrated experience in forming effective relationships and communicating with individuals, groups, and communities of diverse backgrounds, perspectives and cultures
  • Knowledge of population health as well as chronic disease prevention and management and management of the associated risk factors
  • Provide excellent customer service for both internal and external customers.
  • The ability to meet changing deadlines and be able to multitask.
  • Recognize when an issue needs further attention from a supervisor and inform management so that the problems can be addressed.
  • Proficient in written and oral communication
  • Knowledge of programmatic components of public health or social services program implementation.

Job Type: Full-time

Pay: $60,000.00 - $61,000.00 per year
