Purpose

Under the direction of the Healthcare Access ProgramManager, the Healthcare Access Program Coordinator supports the CommunityHealth Access Program (CHAP) through outreach, educational and training opportunitiesand operational administration.

Essential Functions

Program Coordination and Administration

  • Maintain the CHAP database, ensuring appropriate documentation and data collection.
  • Assist in coordination of hiring processes, professional development opportunities and administration of tasks for program staff.
  • Support the Program Manager in conducting regular program audits and implementing quality improvement measures.
  • Develop, maintain, and update policies and procedures to align with funding sources and evidence-based models.
  • Track and document training delivered to CHAP staff.
  • Assist with programmatic reporting and grant writing.

Outreach, Collaboration and Program Promotion

  • Create outreach strategies to strengthen and increase programmatic reach.
  • Assist in the creation, design, and distribution of marketing materials.
  • Coordinate, attend, lead, and present at educational meetings and community events.
  • Establish and maintain collaborative professional relationships with grantor partners, community agencies, and other MHD staff.
  • Oversee systems that document program efforts, client participation, referrals, and follow up.

Application Assistance

  • Assess the needs of walk in customers and assist them in applying to programs for which they qualify (i.e. Badger Care Plus, CORE plan, Family Planning Waiver, and other Medicaid programs). Make referrals to internal or external programs.
  • Track enrolled clients; complete and review data entry.
  • Provide information and outreach to non-traditional populations; provide eligibility information.

Conditions of Employment
  • Standard work hours are 8:00 a.m.-4:45 p.m. Monday through Friday; occasional weekend and evening hours may be required.
  • Must travel between MHD and other community-based sites and/or work at multiple locations.
  • Travel outside the City of Milwaukee for job related training, presentations and conferences; occasional overnight travel may be required.

Minimum Requirements
  1. Bachelor's degree in public health, human services, health education, social work, healthcare management or a closely related field from an accredited college or university.
  2. One year of professional experience in public health, health education, public benefits, social work, human service, community health or healthcare administration performing duties closely related to the essential functions listed above.
  3. Valid driver's license and use of a properly insured vehicle for use on the job at time of appointment and throughout employment. Mileage reimbursement is provided pursuant to Section 350-183 of The Milwaukee Code of Ordinances.

Equivalent combinations of education and experience may alsobe considered; i.e. an associate degree in a health-related field from anaccredited college or university and three years of full-time, relatedexperience as listed in #2 above would be considered equivalent.

Desirable Qualifications
  • Ability to provide interpretation for clients who speak Spanish, Hmong or another language.
  • Knowledge of State or Federal Benefit Program including BadgerCare, Foodshare, Family Planning Only Services, Medicaidor Marketplace.
Knowledge, Skills, Abilities & Other Characteristics

Technical

  • Knowledge of healthcare access, healthcare enrollment and public benefit programs.
  • Ability to read and accurately apply a variety of technical instructions, such as policies, procedures, operating instructions and administrative rules.
  • Ability to use spreadsheet, database, project management and word processing software to develop reports, business correspondence and summary data.
  • Ability to utilize public health data systems and data collection tools.
  • Ability to gather and interpret data and respond to data requests.
  • Ability to monitor staff work activities, assign duties, outline work methods, direct work in progress and check or inspect completed work.

Communication and Interpersonal

  • Written communications skills to create reports and business correspondence.
  • Verbal communication and presentation skills to represent the program effectively.
  • Ability to effectively communicate to a wide variety of audiences.
  • Ability to work cooperatively and fairly with people whose backgrounds may differ from one's own.
  • Ability to work in a collaborative environment with diverse groups of people and at all levels of the organization.
  • Ability to work independently and as a member of highly supportive team.

Current Salary

The current salary range (2FN) is $56,037-$71,754 annually,and the resident incentive salary for City of Milwaukee residents is $57,718-$73,907annually. Appointment above the minimum is possible based upon level ofexperience and other qualifications and is subject to approval.
