This role develops, implements, manages, and evaluates customized, evidence-based, innovative digital and multi-model patient/family education and support tools, projects, and programs that educate, engage and support patients and families. Integrates Supportive Care into tools, projects, and programs in order to expand awareness of supportive care and market programs and resources. Drives integration of resources and tools into patient care.
Recommends, monitors, and reports metrics to assess the impact of digital and multi-modal patient/family education. Analyzes and improves patient and family education processes. and leads and participates in interdisciplinary process improvement initiatives.
Works closely with Supportive Care program managers and director to build relationships across the department and the system in order to advance the Department of Supportive Care Medicine's strategic priorities. Collaborates with Nursing, providers, other clinicians, Patient Communications, Digital Marketing, Digital Innovation, Patient Access and other City of Hope departments as well as with patient and family advisors.
