Job Summary -
This is responsible and specialized case management work in assessing, planning, linking, monitoring, and advocating for a caseload of children/adolescents with serious and persistent mental illness. The incumbent is responsible for assisting clients with their needs/goals through developing the treatment/service plan and/or aftercare plan. Work also entails responsibility for working with their families or guardians, community resources, and advocating for clients by linking them with appropriate services.

Qualifications -
Children/Adolescent Case Management & Aftercare: Bachelor's Degree with major course work in a human services or health education field and one (1) year of full-time experience or a Bachelor's Degree and two (2) years of full-time experience or equivalent experience working with children who have a serious mental illness.
