POSITION SUMMARY:

The full-time Coordinated Entry System Specialist will be a critical member of the Coordinated Assessment team – supporting the overall Coordinated Entry System (CES) for Brevard County. A CES is a centralized or coordinated process designed to coordinate program participant intake assessment and provision of referrals for those experiencing homelessness where the most vulnerable homeless residents are matched with available and appropriate supportive services and housing resources. The position will specifically support the assessment, engagement, enrollment, care coordination, and service provisions for homeless persons while interacting with the CES.

QUALIFICATIONS:

Bachelor's degree in social work, mental health, or human services-related field required. Experience within a Continuum of Care, crisis response system, or homeless services is preferred.

ESSENTIAL JOB FUNCTIONS:

Interviews and conducts standardized assessments of persons experiencing homelessness in-person and virtually while engaging in diversion practices.
Assess immediate housing barriers and help households brainstorm creative options for diverting to alternate housing arrangements while utilizing culturally responsive best practices and skills, motivational interviewing techniques, and/or mediation with a family member or friend.
Completes the CES enrollment process in the Homeless Management Information System (HMIS) by interviewing clients and/or their family members to obtain family, social, employment, medical, and mental health treatment history; provides crisis intervention, motivational interviewing, case management, referrals, and other supportive services to clients and their family members.
Participates in multi-disciplinary team meetings to review client cases, their strengths and barriers to housing, service and housing needs, and appropriate interventions in accordance with the CES’s policies and procedures.
Document screenings, assessments, intakes, referrals, and supportive service transactions in HMIS and maintain at least 95% data accuracy.
Conduct referrals for and coordinate mental health, substance use, and other related services. Understands the institutional barriers to services and assists in connecting households with resources such as childcare, employment, and other mainstream resources that will assist them in obtaining housing.
While the job functions are mostly carried out at fixed office-setting locations in the community, the position could occasionally conduct street outreach and community-based service provision.
