As a 501(c)(3) non-profit organization, River’s Bend operates independently with respect to daily operations, delivery of patient care, and the pursuit of its mission – to provide quality services in the most cost effective manner for the residents, and to provide a stimulating, empowering workplace for our team members. We are currently seeking a MDS Coordinator to join our collaborative team.

The MDS Coordinator is responsible for MDS assesements, Careplans, Bims, PHQ9,and communication between all staff to obtain patient centered care,. Position is responsible for intimate knowledge of Point Click Care and MDS assesments and must be an LPN or RN. Will maintain close interaction with Department Leaders, Community Staff, Administrator and Senior Leadership.
