We are seeking a compassionate and dedicated Hospice Registered Nurse to join our team in Houston and surrounding areas. You will provide specialized care and support to patients with terminal illnesses and their families. Your role will involve providing physical, emotional, and spiritual care to ensure the highest quality of life for patients during their final stages. This is Full-time position.
Responsibilities:
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed by physicians
- Monitor patients' conditions and provide ongoing support and education to patients and their families
- Coordinate with other healthcare professionals to ensure continuity of care
- Provide emotional support and counseling to patients and their families
- Assist with pain management and symptom control
- Document all patient interactions, interventions, and outcomes accurately in medical records
Experience:
To be successful in this role, you should have the following experience:
- Previous experience in Hospice care settings is preferred
- Experience in case management and coordinating care for complex patients
- Proficiency in conducting physical examinations and interpreting medical documentation
- Knowledge of Wellsky and Hospice guidelines
- Ability to provide compassionate care to patients
