We are seeking a compassionate and skilled Hospice Nurse Practitioner to join our team. As a Hospice Nurse Practitioner, you will provide end-of-life care and support to patients and their families during a challenging time.
**Duties:**
Primary functions are to perform face to face encounters and assessments for clients of all ages in their place of residence, coordinate care with the interdisciplinary group, including the hospice medical director/physicians.
- Conduct patient assessments and develop individualized care plans
- Manage pain and symptoms to ensure patient comfort
- Provide emotional support and guidance to patients and their families
- Collaborate with the interdisciplinary team to deliver comprehensive care
- Monitor patient conditions and report changes to the healthcare team
- Assist with end-of-life care planning
Education: Master’s Degree Nurse Practitioner
Licensure: State licensed as a Master’s Degree Nurse Practitioner; State Driver’s License
Experience: Previous experience in hospice, home health, geriatrics or general medical surgical nursing.
Transportation: Reliable transportation.
Health: CPR certified; TB health screen or TB test/CXR according to policy; Hepatitis B Vaccination Consent/Declination
Environmental and Working Conditions:
Works in patients home in various conditions; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; On call. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising the quality of work. And 30-40 miles from the office.
**Skills:**
- Knowledge of psychopharmacology for symptom management
- Experience in critical care settings
- Expertise in pain management strategies
- Familiarity with senior care needs and challenges
- Understanding of ICD-9 coding system
