Job Description

Job Summary:
This position serves as a support to the Medical Records Department to assist in the management of increased vaccination documentation scanning at a Santa Fe, NM medical facility. A minimum one-year (1) commitment is preferred. Options for additional commitment periods exist but are based on client needs.

Job Classification: Full Time Employee 

Compensation:

  • Non-exempt
  • $18-22 per hour 
  • Chloeta reserves the right to offer any candidate above or below the listed compensation amount based on experience or business needs.

Duties/Responsibilities: 

  • Maintains a record of disclosure of all information released. Responds to requests for emergency medical information from hospitals, doctors, medical examiners, law enforcement agencies, and others.
  • Review paper and electronic health records (i.e., scanned notes, reports, special tests, etc.) to identify material to be photocopied/printed/written to electronic media and released.
  • Ensures the information released is limited to what is expressly authorized and to the person or agency designated to receive it.
  • Receives and/or gives out ROI forms and assists visitors and/or callers with completing forms or documents.
  • Accurately enters all requests into the RPMS/ROI section with the necessary information for each request for health information.
  • Identifies, locates, delivers, retrieves, and refiles medical records required for clinical and administrative purposes.
  • Analyzing health records for accuracy, completeness, timeliness, consistency, and compliance (e.g., use of correct titles, linked to correct encounters, etc.) according to HIM policies and procedures.
  • Uses the RPMS/Vista Imaging function to scan records into the EHR.
  • Ensure that the medical records are completed, and all documentation with appropriate documents and signatures are present.
  • Review medical records for missing documents, signatures, and information and notify providers of the deficiency.
  • Review all medical information for sensitive elements to ensure the release is completed only within the required standards and applicable laws.
  • Monitors requests to ensure validity and processing within required time frames.
  • Inquiries vary widely, and the information requires explaining office functions, describing specific requirements, providing basic instructions, or a similar degree of detail.
  • Conducts searches for lost records and completes other special projects and file room activities as assigned.
  • Prepares records for retirement; identifies and pulls those patients' charts, ensuring that all multiple volumes have been secured; boxing charts for shipment; and completing all necessary documentation and record tracking requirements.
  • Requests records from other facilities, including Federal Records Centers, as required.
  • Perform quality review audits on scanned documents.
  • Immediately report to the supervisor any information or circumstances that may
  • violate any statute, policy, or procedure.

Required Skills/Abilities: 

  • Vital customer service, professional communication, and organizational skills.
  • Ability to manage medical information and Quality Assurance of all patient information per HIPAA standards, Privacy Act, and Service Unit and/or Health Center specific policies and protocols.
  • Must adapt and be able to utilize facility information technology systems such as Electronic Health Records (EHR)/ Electronic Medical Records (EMR).
  • Ability to adhere to the local security policies and procedures and appropriate federal required IT security standards.
  • Ensure all required documentation meets the established guidelines of the medical facility, Joint Commission, and CMS, including timeliness, legibility, accuracy, content, date, time, and signature.
  • Flexibility to meet the facility’s scheduling needs.
  • Must be able to pass a federal background check.

Education and Experience:

  • Required: Related customer service experience.
  • Required: 3-5 years of experience in Health Information Management, Release of Information, Medical Records, Medical Billing, Medical Coding.
  • Required: Knowledge and experience using various computer applications, Microsoft Office, Adobe, and conferencing software.
  • Required: High school diploma or GED.
  • Required: Must possess a valid state driver’s license throughout the term of this contract.
  • Preferred: Medical supply experience
  • License verification, education, and certifications must be provided at the time of application and can be emailed to certification@chloeta.com.

Physical Requirements:

  • Able to frequently lift at least 15 lbs., reach, pull, push, bend, stoop, and kneel. 
  • Prolonged sitting at a desk and working on a computer and standing/walking.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements: 

This position may be contingent upon passing a security clearance, criminal background check, drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position. 

Drug and Alcohol Testing:
To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process. 

Fair Credit Reporting Act (FCRA):
Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
