CHLOETA is Hiring a Case Manager Registered Nurse (RN) Near Crownpoint, NM

Job Description:


Job Summary:

Chloeta is looking for a Registered Nurse Case Manager to provide nursing services in the delivery of patient care at an Indian Health Services (IHS) Facility in Crownpoint, New Mexico. The RN Case Manager will be focused on providing effective and efficient medical care while managing the costs of treatment, as well as acting as a liaison between patients, their families, doctors, and other health care providers.

Job Classification: Full Time Contract

Supervisory Responsibilities: No

Duties/Responsibilities:

  • Contractor shall provide full range of adult and pediatric case management services in the Ambulatory Care and/or Women and Children's services.
  • Contractor shall exercise knowledge of the nursing process and its application to clinical practice to provide individualized, goal-oriented nursing care to the adult/pediatric patient.
  • Develop, implement, and evaluate the service unit patient care coordination and case management program for nursing services to comply with current standards of care.
  • Collaborate with primary care providers, clinic nurses, public health nursing, case management and social services to ensure that the patient care needs are done timely and cost effective.
  • Manages all clinic visits and follow-up visits through coordination with multi-disciplinary teams of physicians, nursing personnel, social workers, physical therapists, etc. accomplish this, and chart reviews.
  • Arranges and facilities patient-care conference with providers, patients, and/or patient's family as needs. Interprets for patient's family and providers where applicable.
  • Monitors and evaluates the effectiveness of the patient care coordination process by reviewing charts of patients to determine if documentation is present.
  • Provides information to providers based on the diagnoses and implements treatment plan according to age-appropriate health maintenance needs and standards of care. Assists providers and patients/families in meeting treatment plan by coordinating appointments to meet the expected clinical outcome.
  • Participates in patient care conferences between various disciplines, families, and agencies to determine the patient's plan of care. Utilizes various patient education resources to assist patient/families in understanding, accepting, adjusting to diagnoses/treatment modalities and their role responsibilities. interprets for patients, family and providers when applicable.
  • Collaboratively works with Purchased & Referred Care (PRC) staff for necessary follow-up on outside testing and obtaining results for the ordering provider. Participates in the primary care team planning and coordination rounds.
  • Provide appropriate documentation regarding care rendered using Crownpoint Service Unit (CPSU) Electronic Health Record (EHR) & RPMS system. All documentation by the provider shall be entered electronically using the Electronic Health Record system. The provider is responsible for utilizing the EHR/RPMS system to obtain pertinent patient information in assisting care. It is the responsibility of the provider to use the training resources provided to develop competency in utilizing the Electronic Health Record system/RPMS.
  • Other duties as assigned.

Education and Experience:

  • Required: Current, valid, unrestricted nursing license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States throughout the term of employment.
  • Required: An associate's (or higher level of education) degree in nursing from an accredited school in the U.S.A.
  • Required: Must have at least 3 years of experience as a RN with 2 years in the specialty.
  • Current Basic Life Support (BLS) is mandatory for all specialties.
  • Pediatric Advanced Life Support (PALS), Newborn Life Support (NLS), Advanced Trauma Care Nurse (ATCN) or Trauma Nurse Critical Care (TNCC), and Advanced Cardiac Life Support (ACLS) are recommended for each appropriate duty section at the time of selection for contract award.
  • The contractor shall ensure that the contract nurse's certifications remain valid throughout the term of this contract.
  • Required: Provide three (3) professional references at the time of application.
  • Required: Must be able to pass a federal background check.
  • Licenses, CV/resume, references, and certifications must be provided promptly at the time of application and can be emailed to certification@chloeta.com.

Physical Requirements:

  • Must be able to withstand physical and emotional stress.
  • Must be able to stand and move for long periods of time.
  • Able to frequently lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:

This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.

Drug and Alcohol Testing:

To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.

Fair Credit Reporting Act (FCRA):

Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
