What You Provide Us :

• Meets with patients for one-on-one support and guidance
• Assists patients with developing recovery plans, coping methods, and rehabilitation strategies with patient and behavioral health team
• Monitors patient's behavior, schedule, medication, and activities to report back to behavioral health team
• Liaising with family members and medical professionals regularly
• Generates recovery reviews and performance documents to track the rehabilitation process
• Assists with arranging medical appointments, therapy sessions, support groups, and other resources to aid the patient as indicated by the patient

Must Haves :

• High School Diploma or GED required
• Experience in community engagement, with emphasis on building & maintaining partnerships
• Valid Fingerprint Clearance Card
• Any combination of experience and education that provides the required skills may be considered
