Details:

• Meet with patients for one-on-one support and guidance.
• Assist patients in developing recovery plans, copingmethods, and rehabilitation strategies in collaboration with the behavioralhealth team.
• Monitor patients' behavior, schedules, medications, andactivities, providing feedback to the behavioral health team.
• Liaise regularly with family members and medicalprofessionals.
• Generate recovery reviews and performance documents to trackthe rehabilitation process.
• Assist in arranging medical appointments, therapy sessions,support groups, and other resources based on patient needs.

Requirements:

• High School Diploma or GED required.
• Completed Peer Support Training with a valid certificate.
• Experience in community engagement, with an emphasis onbuilding and maintaining partnerships.
• Valid Fingerprint Clearance Card.
• Any combination of experience and education that providesthe required skills may be considered.
