Summary: This position is responsible for coordinating hospital and community resources in accordance with treatment team recommendations. Assist with resource applications, facilitating ongoing governmental and community aide in collaboration with unit based clinical social worker. Responsibilities:
Provide information and referral on available community resources identified by the clinical social worker. Initiate contact with family for prompt follow-up on needs (For example, concrete resources such as housing, transportation, food, outpatient counseling, inpatient community behavioral health treatment, wish organizations, disability applications, etc.)
Assist with the resource and referral process involved with the ongoing assessment of Social Determinants of Health (SDOH)
Liaison with outside agencies to maximize follow through by agency and family
Collaborate with unit based social worker and clinical stakeholders on availability of support to meet the identified needs of the family
Document resource assistance and referrals in departmental logs and in the Electronic Medical Record (EMR)
Update clinical social worker on progress in locating resources
Provide updates and guidance to families regarding referrals and disposition
Perform other duties and responsibilities as assigned
WORK EXPERIENCE
At least 2 years in Human Services related field (Human Development, Family Studies, Social Work, Psychology) Pref
EDUCATION
High school diploma or equivalent Req
Two-year Associate's degree or equivalent experience Human Services or related field Pref
