The position of Case Manager/Home Visitor will pay $26/hour

Additionally, the lifespan compensation range of this position is:

$26/hour - $31.25/hour

The Case Manager/Home Visitor (CM/HV) is responsible for implementing a home-based prevention effort for moderate- to very high-risk families with children 0 -5 years. The CM/HV assists families in establishing a strong bond with their baby, understanding their child's development, and keeping their child health and safe. In addition, the CM/HV supports families with referrals to outside agencies, goal planning and improved family functioning strategies. The CM/HV is responsible for completing detailed documentation on a weekly basis for all the services provided.

The CM/HV may assist families in crisis to assess resources to meet their needs, coordinating information for participant referrals, compiling and maintaining records, preparing reports and attending all required meetings. Position may be located at a Family Resource Center.

Duties and Responsibilities:

Prevention Efforts

  • Conduct assessments and screenings to assess client needs, developmental milestones, depression and domestic violence.
  • Make referrals to other community agencies and assist families to reach resources within the community.
  • Aid families in crisis situations such as immediate need for shelter, childcare, health care, etc. by providing proper referral and case management.
  • Responsible for assigned programs for children and/or adults such as parent support and education.
  • Work directly with families to provide basic preventative health care information or to assist with budgeting, home organization, and other skills necessary for improved safety and management.
  • Develops and maintains community resource files. Works with the Information and Referral staff to provide proper referrals to the families.
  • Translates materials for clients and the agency/collaboration.
  • Attends community meetings and/or agency collaboration meetings as assigned.
  • Maintains case notes, contact logs, and other records as required by the agency and funding source.
  • Support families and individuals in home or in office in establishing and meeting their goals.
  • Teach and model healthy parent-child interaction in the home using a trauma informed /empathetic approach and curriculum.
  • Schedule, confirm and conduct weekly or biweekly home visits with clients
  • Establish a trusting relationship with clients.
  • Deliver research-based information on health and safety, nutrition, family functioning and child development!
  • Assist families in identifying/reaching appropriate community resources
  • Attend team meetings and weekly individual reflective supervision meetings

Case Management

  • Meets service provision requirements for targeted case management.
  • Meets on-going comprehensive case management requirements.
  • Conduct and document needs assessment.
  • Development of a Written Individualized Service Plan
  • Linkage, Consultation, and Referral including follow-up.
  • Assistance with Accessing Services
  • Crisis Assistance Planning
  • Periodic Review

Documentation

  • Maintain case files and notes (including releases, etc.)
  • Maintain work and activity sheets, statistics, court reports/letters, as required
  • Meets documentation requirements in relation to contact logs

Community

  • Maintain/provide current resource referrals within the community.
  • Develop/maintain effective working relations with all individuals and agencies impacting program within the community.
  • Assist Program Supervisor with developing and implementing community outreach programs.

General

  • Maintain confidentiality in matters of agency operations, personnel, and clients
  • Maintain positive communications with all agency staff, clients, and volunteers
  • Exercise good judgment in performance of duties and responsibilities
  • Support existing agency policies, principles, and mission

What you will need to bring:

  • Master's Degree in Social Work, Psychology, Marriage and Family Counseling or a closely related field.
  • Two or more years of experience in working with families and children 0 - 5 years old, or in a supportive capacity to parents.
  • Experience with community outreach to low-income and/or moderate- to high-risk families.
  • Knowledge of early child development and community resources desirable
  • Ability to conduct assessments and developmental screenings.
  • Ability to work in a multi-cultural environment.
  • Bilingual English/Spanish is required
  • Good organizational, follow-through and verbal/written communication skills
  • Ability to establish healthy boundaries and trusting partnerships with families.
  • Home visitation experience preferred.
  • Ability to work collaboratively with community organizations
