POSITION SUMMARY

The Community Services Care Coordinator partners with children and families in our community helping to manage and coordinate medical and mental health care and other services and supports. Care Coordinators monitor and coordinate client care coordination/treatment plans, educate them about their condition(s), connect them with service providers, and evaluate their progress.

This position helps clients by identifying service needs and assisting with the utilization of service providers and community resources that strengthen and enhance the quality of their lives.

MINIMUM QUALIFICATIONS AND COMPETENCIES

• Bilingual, English and Spanish, preferable;
• Bachelor’s degree in behavioral health or social service-related field, required andtwo (2) years behavioral health or social service work experience; or
• Bachelor’s degree in non-behavioral health or social service-related field and four (4) years behavioral health or social service work experience;
• Fingerprint Clearance Card;
• Pass Centralized Screening;
• Ability to travel when needed; and
• Supervisory or management experience preferred.

Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking, and other requirements established by CPLC from time to time.
