The Health Care Navigator plays a pivotal role in providing essential support to veterans and their families, ensuring access to health care services and fostering self-directed recovery. This position requires a strong background in the human services field, along with excellent communication skills to effectively collaborate with various community partners and support systems. The navigator assists veterans in optimizing treatment interventions, accessing community services, and adhering to health care plans while upholding Chestnut’s culture of customer service excellence.
Candidates for this role should possess a minimum of a high school diploma with 20 years of experience in the human services field, military service, or related administrative area. Alternatively, a Bachelor’s degree in a helping profession or related administrative area with 10 years of relevant experience, or a Master’s degree with 5 years of experience, is required. Preference is given to military veterans or immediate family members of veterans. Strong telephone skills, professional communication abilities, and proficiency in Microsoft Office tools and electronic health records are essential. A valid driver’s license, private automobile insurance, and basic computer skills are necessary for this position.
